Fire hydrants must be of sufficient number and so located that any part of the vessel, other than main machinery spaces, accessible to persons on board while the vessel is being navigated and all cargo holds may be reached with at least 2 streams of water from separate outlets, at least one of which must be from a single length of hose. In main machinery spaces, all portions of such spaces must be capable of being reached by at least 2 streams of water. One of the two required vessel fire stations found inoperable. No pressure shown at fire station after fire pump was energized and isolation valve opened. 46 CFR 95.10-10

...Nozzle required to be approved under the provisions of this subpart must be of brass or bronze... Fire hose nozzles were not of an approved type; constructed of plastic. 46 CFR 95.10-10(i) 46 CFR 162.027

Scarf and weld the cracked weld starboard of the towing bit at the bow.

Prove proper operation of the Z-drive indicator on the stbd engine.

Provide retroreflective tape on all ring buoys.

Affix placard above indicating light on bridge for the forward generator that indicates "low pressure."

Prove proper operation of both schottel propulsion drive units. This required testing is to include verification that no oil leaks from either of the schottel units during all modes of operation. _x001C_

Provide current cargo gear certificate/register for cargo boom on the aft deck (46 CFR 91.25-25 (b)(c) & (d). Vessel is prohibited from using cargo boom until cleared.

Provide proof of enrollment of all crew members in a drug/alcohol testing program meeting 46 CFR 16.

Provide proof of licensed radio operator in the vessel's crew.

Have entire crew conduct fire and man overboard drills in presence of an attending marine inspector.

Have 15 lb CO2 portable fire extinguisher, located in the pump room, hydrostatically tested. (This is an extra fire extinguisher).

MAKE PERMANENT REPAIRS TO MAN OVERBOARD RESCUE LADDER.

REPLACE LINE ATTACHED TO LIFERING LIGHT ON BRIDGE DECK. LINE IS WASTED.

HAVE ALL CARGO PIPING HYDROSTATICALLY TESTED.

STENCIL ALL LIFEJACKETS WITH VESSEL'S NAME.

CALIBRATE AIR RECEIVER AND CARGO PUMP GAUGES PER 46 CFR 61.

CLEAN ALL OIL OUT OF LAZARETTE SPACE. DETERMINE CAUSE OF OIL LEAKAGE AND REPAIR.

PROVE PROPER OPERATION OF SAFETY RELIEF VALVES ON AIR RECEIVER. ONE OF TWO VALVES WOULD NOT HAND RELIEVE PER 46 CFR 61.10.

PROVIDE DATA PLATE INFORMATION FOR ALL CARGO PUMPS.

PROVIDE CARGO GEAR/CRANE TESTING CERTIFICATE AND LOAD TEST DATA TO MARINE INSPECTOR. PROVE PROPER OPERATION OF CARGO GEAR PRIOR TO USE.

REVIEW TRANSFER PROCEDURES FOR VESSEL AND ENSURE COMPLIANCE WITH 33 CFR 155.720. PROVIDE SHIP SPECIFIC INFORMATION IN FORM AVAILABLE TO PERSON IN CHARGE.

PROVIDE TEST DATA FOR CARGO PIPING AND PROPERLY STENCIL THE CARGO PIPE (WITH TEST DATE).

PROVIDE THE FOLLOWING DOCUMENTS: RENEWAL STICKER FOR CERT. OF DOCUMENTATION, FCC RADIO OPERATORS LICENSE, FCC RADIOTELEGRAPHY SAFETY CERTIFICATE.

PROVIDE IMPROVED MEANS TO PICK UP A PERSON IN THE WATER.

PROVIDE APPROVED VESSEL RESPONSE PLAN AND COAST GUARD APPROVAL LETTER AS REQ BY 33CFR155.1045._x001C_ _x001C_ MUST BE PROVIDED PRIOR TO CARRYING OIL AS CARGO.

CONDUCT CONDUCTIVITY TEST OF HOSES AND NOZZLES FOR ALL CO2 BOTTLES AND SEMI-PORTABLE SYSTEM.
