Personal equipment shall consist of the following: protective clothing of material to protect skin from the heat radiating from the fire and from burns and scalding by steam. The outer surface shall be water resistant. Protective equipment did not sufficiently cover the ear area and portions of skin on face.

The Company shall ensure that: Valid documents are available at relevant locations, Changes to documents are reviewed and approved by authorized personel, and Obsolete documents are promptly removed. PSCO observed during Abandon Ship Drill that a crew member was unable to don a life jacket, and the use of two conflicting muster and abandon ship documents.
