Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. No lights found on any adult or child life jackets. Attached approved lights to front shoulder of all lifejackets. Vessels route is reduced to no more than 20 miles from a harbor of safe refuge (limited coastwise) until resolved to the satisfaction of the attending marine inspector. 46 CFR 180.75(a)

No marine employer may engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. One new employee/crew member has not had pre employment testing. Provide evidence of pre employment testing for new crew member before March 11, 2026, to the satisfaction of the attending marine inspector. 46 CFR 16.210(a)

A vessel of more than 19.8 meters (65 feet) in length must carry a minimum of three life buoys of not less than 610 millimeters (24 inches) in diameter. 2 ring life buoys are deteriorated and no longer serviceable Replace 2 24 inch life ring buoys with 46 CFR 160.050 approval before March 13, 2026. 46 CFR 180.70(a)(3)

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Unapproved blade type fuse found on battery underneath helm station. Replace with UL approved cartridge type fuse. 46 CFR 183.380(l)

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Head tank for raw water wash down system is not permanently attached to bulkhead. Permanently attach head tank to bulkhead. 46 CFR 176.800(a)

Each garbage management plan under paragraph (a) of this section must be in writing and:(1)Provide for the discharge of garbage by means that meet Annex V of MARPOL, the Act, and §§ 151.51 through 151.77; (2)Describe procedures for minimizing, collecting, processing, storing, and discharging garbage; and (3) Designate the person who is in charge of carrying out the plan. No garbage management plan on vessel. Create and post garbage management plan. 33 CFR 151.57(b)

The requirements for pressure vessels by class are as follows: Classes II and III pressure vessels that have a net internal volume of less than 0.14 cubic meters (5 cubic feet) and do not contain hazardous materials must be stamped with either the ASME “U” or “UM” symbol. Compliance with other provisions in this part is not required. Pressure vessel serving toilets is not stamped with ASME "U" or "UM" symbol. Submit current pressure vessel for ASME approval or procure new ASME approved pressure vessel. 46 CFR 54.01-5(c)(4)

The generator circuit breakers must be interlocked to prevent the generators from being simultaneously connected to the switchboard, except for the circuit breakers of a generator operated in parallel with another generator when the installation meets §§ 111.12-11(f) and 111.30-25(d) in subchapter J of this chapter. Current arrangement does not provide effective means of testing reverse power relays on generators. Provide procedures for testing reverse power relays prior to paralleling generators or remove ability to parallel generators and test for Coast Guard. 46 CFR 183.322(b)

Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel failed to complete second annual in required window. Complete a renewal examination to the satisfaction of the attending marine inspector. 46 CFR 176.500(b)(1)

Marine Inspector observed, Top bar on deck rail gate, starboard side, was damaged. Repair or replace rail to the satisfaction of the attending Marine Inspector. 46 CFR 176.800(a)

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter in this subchapter may be inspected in accordanxe with standars acceptable to the cognizant OCMI as good practice. Vessel POC informed Marine Inspector that shaft bearing will be replaced. Replace shaft bearing to the satisfaction of the attending Marine Inspector, prior to credit drydock.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel was due to conduct drydock by 30JUN25 and failed to within alloted time frame. 46 CFR 176.500(c)(1)

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. The required fire extinguishers and pre-engineered extinguishing system are not within their 12 month servicing window as required by NFPA 10. 46CFR176.810(b)(1)

The OCMI may require the vessel to get underway. Get underway in presence of marine inspector to conduct drills. 46 CFR 176.404(a)
