After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's stability letter. The vessel may not depart until it is in compliance with these requirements. The vessel master was unfamiliar with how to perform loading calculations in accordance with the vessels stability letter and approved operations manual. Company shall ensure all masters responsible for the stability of the vessel is adequately trained to perform necessary stability loading calculations. 46 CFR 185.315(a)

Abandon ship drills and training shall be logged or otherwise documented. The drill entry shall include the date of the drill and training and the general description of the drill scenario and training topics. The vessels master could not provide logs or evidence of man overboard or abandon ship drills and training as required by regulation. Vessel completed proper drills to the satisfaction of the Coast Guard. Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection and one at the end of the fuel line to stop fuel flow when servicing accessories. During inspection it was discovered that the emergency fuel shut off valve was located approximately 10' from the valve installed at the fuel tank. Vessel shall alter placement of emergency fuel shut off valve to be located at the closest valve to the fuel tanks at next drydock. 46 CFR 182.20-45(b)(2)
