SUPERIOR FORTITUDE

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's stability letter. The vessel may not depart until it is in compliance with these requirements. The vessel master was unfamiliar with how to perform loading calculations in accordance with the vessels stability letter and approved operations manual. Company shall ensure all masters responsible for the stability of the vessel is adequately trained to perform necessary stability loading calculations. 46 CFR 185.315(a)

The master shall conduct sufficient drills and give sufficient instruction to make sure that all crew members are familiar with their duties during emergencies that necessitate abandon ship or the recovery of persons who have fallen overboard. Abandon ship drills and training shall be logged or otherwise documented. The drill entry shall include the date of the drill and training and the general description of the drill scenario and training topics. The vessels master could not provide logs or evidence of man overboard or abandon ship drills and training as required by regulation. Vessel completed proper drills to the satisfaction of the Coast Guard. Vessel shall perform training on emergency procedures and provide documentation to the Coast Guard prior to departure. 46 185.420(a),(c) & (f)

The Master of the vessel shall ensure that: Each EPIRB must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel was missing required monthly EPIRB test in the vessel log. Vessel shall conduct required monthly test of EPIRB and log it in the vessel log book. 46 CFR 185.728(c)

The Master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Grab lines for stern life ring buoy was in a deteriorated condition. Vessel shall replace grab line on stern life ring buoy. Additionally the line throw bag had string wrapped around the opening, preventing immediate use. 46 CFR 141.230

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts. The vessels chart for area of operation was printed on October 2019 and did not contain any chart corrections until week 36/20. Also the vessel latest Notice to Mariner was not on board. Vessel must ensure charts are maintained continuously throughout the validity of the vessels COI. 46 CFR 184.420

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection and one at the end of the fuel line to stop fuel flow when servicing accessories. During inspection it was discovered that the emergency fuel shut off valve was located approximately 10' from the valve installed at the fuel tank. Vessel shall alter placement of emergency fuel shut off valve to be located at the closest valve to the fuel tanks at next drydock. 46 CFR 182.20-45(b)(2)

Missing current charts for areas of operation. Charts onboard were from 2018.

Missing documentation in log book of monthly EPIRB test.

Missing documentation in log book of fire, abandon ship, and MOB drills being conducted.

Captain could not provide proof of a valid Marine Radio Operator Permit.

Port ring buoy 90' bag of line was wasted.

Test or replace safety relief valve on pressure vessel.

Install additional wrapping on keel cooler piping.

Install additional wrapping on stbd MDE exhaust.

Port lifebuoy line was wasted. Required new line be installed. 46 180.70(a)(b)(1)

The jacking system should incorportate a "dead man" device at the system controls to secure jacking if applied force to controls is removed. Device used would not secure jacking. Once foot pedal pressure was removed the hydraulic plate used to return controls to "off" would not move levers to secure jacking operation. CG 543 Policy Ltr Enc 1, Para 11.b

Fuel tank vent screen 30x30 mesh was holed from check valve wearing on screen. 46 CFR 182.450

Epirb battery was expired on 7/2018. 46 CFR 180.64

EPIRB Hydrostatic release was expired from 2017. 46 CFR 185.740 .

Life float is required 2 paddles. Vessel's life float was missing both. 46 CFR 180.175(d)

Vessel is required to have a rescue boat due to the nature of its work making it restricted in its ability to maneuver. Rescue boat engine was inoperable to to a fuel filter leak. 46 CFR 180.210(b)(2)

Fire extinguisher mounted to protect pain locker had a wasted bracket. Requires replacement. 46 CFR 181.500(a)

Magnetic compass is required a light unless vessel is operating only in daylight. Vessel operates 24/7 and requires light. 46 CFR 184.402(c)

Remote fuel shut off vavle on main deck did not have directions showing how to operate. 46 CFR 182.455(b)(4)

Emergency escape hatch leading from Engine room was unable to be opened from the inside. Crew to adjust hatch. 46 CFR 46 177.15-1

PFD was missing the required attached light.
