A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Marine inspector observed the the FCC Station License MMSI number did not match what was programmed into the VHF radio. Also the call sign for the vessel was not the same on the Station License as it was on the Radiotelephony Certificate. The Radiotelephony Certificate was for the operator's last vessel. Vessel must correct all FCC registration documentation to reflect most current information. The Radiotelephony Certificate must be valid and be for the MAGIC DOLPHIN III. Vessel must also ensure the information is accurately documented on the EPIRB registration.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. MI observed alcohol testing devices on board to be expired. Replace alcohol testing devices to the satisfaction of MI.
