PER THE SMS PROCEDURES ON BOARD (ISM MANUAL PART I, CH3-PERSONNEL) CREW WAS NOT FAMILIAR WITH KEY SYSTEMS ON BOARD. FOLLOWING ITEMS WERE OBSERVED: 1. DURING FIRE DRILL TEAM LEADER AND FIRE ATTACK TEAM NO. 2 WAS NOT FAMILIAR WITH SCBA SYSTEM BY CREW.
2. ENGINEERING CREW WAS NOT FULLY FAMILIAR WITH IG SYSTEM ON BOARD, TO INCLUDE ALARM PARAMETERS.
3. DECK CREW WAS NOT FULLY FAMILIAR WITH PORTABLE GAS DETECTION SYSTEM CALIBRATION.
REQUIRE EXTERNAL AUDIT WITHIN 90 DAYS.
