The operating levers of quarter-turn valves must be parallel to the fluid flow when open and perpendicular to the fluid flow when closed. MI observed the bilge system valves were inoperable. Provide a risk mitigation plan in regards to the bilge holding tanks until proper repairs are effective. 46 CFR 56.20-9(a) 16C

Each tank must be provided with a suitable means of determining liquid level. MI observed the level indicating device was not attached nor calibrated. 46 CFR 56.50-90(a) 16C

No person may use a vessel to which this part applies unless:(1) It has a number issued on a certificate of number by the issuing authority in the State of principal operation; and(2) The number is displayed as described in § 173.27. MI observed an expired State Registration. 33 CFR 173.15 705C (7days)

Any vessel of at least five net tons wholly owned by a citizen or citizens of the United States is eligible for documentation under this part. This includes, but is not limited to, vessels used exclusively for recreational purposes and vessels used in foreign trade. The vessels's COD was expired. 46 CFR 67.5 705 C (Prior to Operation)

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. MI observed that the vessel was overdue for annual inspection. 46 CFR 31.10-17(a)

All lifesaving appliances and arrangements on tank vessels must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Each vessel must carry lifebuoys approved under approval series 160.150. Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port. At least one lifebuoy on each side of the vessel fitted with a buoyant lifeline that is— (A) 30 meters (100 feet) in length; (B) Non-kinking; (C) Not less than 8 millimeters (5/16 inch) in diameter. Vessel did not have a Lifebuoy onboard. 46 CFR 31.36-1 46 CFR 199.70

A ship, except a ship of less than 26 feet in length, must have a placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station, stating the following: Discharge of Oil Prohibited. Vessel did not have placards onboard. 33 CFR 155.450

The master or person in charge of a ship to which this section applies shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. These locations must be readily accessible to the intended reader and may include embarkation points, food service facilities, garbage handling spaces, living spaces, and common areas on deck. Vessel did not have garbage discharge placards onboard. 33 CFR 151.59

The master shall ensure that the following information for each cargo carried under this part is readily available to those on the tankship engaged in cargo operations: (1) The name of the cargo as listed in table 1. (2) A description of the cargo's appearance and color. (3) Hazards in handling the cargo. (4) Any special handling procedures for the cargo, such as inerting. (5) Procedures to follow if the cargo spills or leaks. (6) Procedures for treating a person exposed to the cargo. (7) A list of fire fighting procedures and extinguishing agents effective with cargo fires. (8) Shipper's name. (9) Loading point. (10) Approximate quantity of cargo. (11) Tank in which the cargo is located. (12) The name of an agent in the United States authorized to accept service of legal process for the vessel. Vessel did not have Safety Data Sheet onboard. 46 CFR 153.907 04 16 C
