Nonconductive mats or grating must be provided on the deck in front of each switchboard and, if it is accessible from the rear, on the deck behind the switchboard. Numerous high voltage switchboards and electrical panels in engine room do not have nonconductive mats provided. 46 CFR 143.560(c)

A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. Towing gear inspections are not being logged as required. 46 CFR 140.820(b)

Flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use...A 40-B portable fire extinguisher must be located near the storage room or cabinet. Combustible liquids are being stowed in a room on the main deck without a fire extinguisher nearby.

Fixed fire-extinguishing system: (1) A carbon dioxide system that meets the requirements of 46 CFR sub part 76.15 and 46 CFR 78.47-9 and 78.47-11, and that is approved by the commandant... Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. On systems in which the CO2 cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Each control valve to branch lines must be marked to indicate the related space served.

Flexible connections and discharge hoses on all fixed extinguishing systems must be inspected and tested in accordance with § 147.65 of this chapter. CO2 hoses failed most recent inspection. Replace hoses.

All piping each fixed fire-extinguishing system must be tested, to determine that the system is operating properly. Conduct air test on fixed fire system.

A 40–B portable fire extinguisher must be located near the flammable storage room or cabinet. Extinguisher was not near flam cabinet on deck.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and must be either: Automatic, battery-operated with a duration of no less than 2 hours; or Non-electric, phosphorescent adhesive lighting strips that are installed along escape routes and sufficiently visible to enable egress with no power. Berthing spaces did not have lights and the light in shaft alley inop.

Electrical components that pose an electrical hazard must be in an enclosure. Cap or remove wires that are not in use.

Each emergency exit used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”.

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. The navigation assessment must incorporate the requirements of pilothouse resource management of § 140.640, assess operational risks, and anticipate and manage workload demands. At a minimum, this assessment must consider the items listed in 46 CFR 140.635.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. The gaskets on the hatch cover to the deck voids were wasted. Replace gaskets.
