Marine Inspectors (MIs) observed square inserts and pipe/drain penetrations located in the port and stbd forward bow thruster void space in the under-deck that were not welded on the under-deck side.

Vessel reported a hydraulic plug failure in the center Z-Drive causing a discharge of oil.
Identify any issues with the Z-Drive and provide this office with a plan for repair if determined that Z-Drive is inoperable.
Make appropriate repairs to the satisfaction of the cognizant OCMI. 46 CFR 119.200
