Ships carrying toxic or flammable products or both shall be equipped with at least two instruments designed and calibrated for testing the specific vapours in question. If such instruments are not capable of testing for both toxic concentrations and flammable concentrations, then two separate sets of instruments shall be provided.
Vessel does not have proper tubes for detection equipment on board to be able to test for toxic vapours.

Electrical equipment cables and wiring shall not be installed in the hazardous locations unless it conforms with the standards not inferior to those acceptable to the Organization. Electrical wiring was exposed on explosion proof light fixture above #6 starboard cargo tank.

A tank overflow control system required by this section shall come into operation when the normal tank loading procedures fail to stop the tank liquid level exceeding the normal full condition and give a visual and audible tank overflow alarm to the ships operator.
The #6 starboard tank overfill audible and visual alarm are not functioning.

Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern.
One vapor connection line on port and starboard side are missing the permanent stud.

The master, owner, operator, agent, or person in charge of a vessel subject to this subpart and this section must submit a ballast water report to the National Ballast Information Clearinghouse (NBIC) by electronic ballast water report format using methods specified at NBIC's Web site.
Vessel failed to submit a ballast water report to the NBIC 6 hours after entering port.
