Vessel's fixed fire system is required to be serviceable condition. System was inspected by a third party & found to be deficient. Once fire system is deemed serviceable by third party inspector send report to attending MI.

Each towing vessel must carry at least one appropriately-sized lifejacket, approved under approval series 160.002, 160.005, 160.055, 160.155, or 160.176, for each person on board.

Fuel shut off vaves must be installed to stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. The valve must be installed in the fuel piping directly outside of the fuel oil supply tank. The valve must be operable from a safe place outside the space where the valve is installed. Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it.

Vessel must be fitted with a general alarm that: Is activated at each operating station and can notify persons onboard in the event of an emergency; is capable of notifying persons in any accommodation, work space, and the engine room; Has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: “Attention General Alarm—When Alarm Sounds or Flashes Go to Your Station”

Vessel's towing gear for pushing ahead or towing along- side to be frequently inspected & serviceable. Vessel did not have a towing gear inspections log.

The health, safety, and operational requirements for towing vessels and the crew members serving on-board them requires each vessel to have a safety and health manual and training, TVR, fire and man over board drills,training, safety orientation for crew and non-crew,navigation assessments, examinations and tests of steering, nav, communication, propulsion, towing, signal whistle.

Vessel is required to have all alarms & monitoring for MDE's, generators, & steering. Vessel does not have installed all applicable alarms at the operating station

Vessel is required to have an illuminated magnetic compass or swing meter. Install compass or swing meter.

Vessel is to have the appropriate amount of VDS (flares) in the approved container w/the applicable markings. Vessel does not have VDS, container, or markings.

Vessel is required to have an industrial first aid cabinet or kit. Vessel does not have appropriate kit.

Vessel is required to have an additional 40-B fire ext. in the vicinity of the haz locker. Vessel does not have the prescribed extinguisher near the haz locker.

Vessel is required to have a fuel maintenance plan. Vsl doesn't have the prescribed fuel maintenance plan. Provide proof of plan to attending Marine Inspector. Vessel's operator is required to have a Marine Operator permit (radio's) or higher. Vessel's operator did not have appropriate permit.

Vessel is required to have an Employee Assistance Program (EAP) / Drug & Alcohol Program (DAPI) IAW w/the prescribed regulations. Vessel is also to have adequate means of testing for presence of alcohol (strips) within 2 hours of casualty. Vessel does not have the alcohol strips & crew submits to ACOE drug testing requirements which do not meet the 50% requirement for mariners.

Vessel's piping for flammable liquids & water cooling must be fitted w/isolation valves & clearly marked. Vessel's piping are not marked / color-coded.

Vessel's escape hatches & emergency exits to be marked Emergency Exit, Keep Clear. Vessel's exits were not appropriately marked.
