The safety management system should include procedures ensuring that non-conformities, accidents, and hazardous situations are reported to the Company, investigated, and analyzed with the objective of improving safety and pollution prevention. Objective evidence at the time of inspection indicates the following timelines for material condition nonconformities identified in deficiencies 2-4: 1. Fuel tank found at 1/8 tank at time of inspection. 1A. 3A/E - stated the tank level remained the same for the duration of the time he had been onboard (approx. 2 months). 1B. The insufficient fuel tank level was identified in the rescue boat servicing report dated 8Nov2023. 1C. Rescue boat fuel was not ordered until the COI annual (18Nov2023). 1D. Rescue boat was incapable of meeting SOLAS operability requirements for at least 2 months. 1E. No weekly/monthly maintenance checklist has been established for the rescue boat since reflag in May2023. 1F. Rescue Boat weekly check comments on 9Sep23, within the job history indicate that “fuel checked”, noting no fuel level deficiency. 2. Throttle cable degraded material condition. 2A. Degraded condition discovered OOA 19Oct2023 by crew and removed from the rescue boat for troubleshooting, degrading the operability of the rescue boat from 19Oct2023 – 19Nov2023. 2B. Degraded condition identified in the rescue boat servicing report dated 8Nov2023; required replacement. 2C. The report to USCG was made in preparation of the COI annual (15Nov2023) and not at the time the rescue boat was reduced in its operational capacity (27 days prior). 3. Impeller reduced operability/inoberability. 3A. Impeller identified as deficient, reported via email to company, and requested service/replacement OOA 28June2023. 3B. SR-698-2023R-0210 filed within SMS maintenance system on 28June2023; no resolution/ordering of parts prior to the third party inspection conducted on 8Nov2023. 3C. Conflicting comments of Rescue Boat condition within the weekly test reports following 17Jun23 indicate the second engineer was completing inspections and maintenance, noting no deficiencies on the equipment, despite previous reports from the C/E. 3D. Rescue boat identified as “inoperable” during the company internal SMS audit on 26Sep2023 and not immediately addressed or reported to the USCG. 3E. Impeller inoperability identified in the technician’s report dated 8Nov2023, required replacement. Parts ordered thereafter. 3F. On 01Oct2023, the “Rescue Boat Port Engine Weekly Testing” Job (Job Code: H720-501-MA-TS-0193) was marked “Postponed” within the TOUCH system. The weekly test was then removed from the periodic test timetable and not completed thereafter. The Marine Inspector found 28 postponed jobs within the TOUCH system, that can not be manipulated by the vessel’s crew. More: See attached 835 and Narative.
