The owner of a vessel shall ensure the vessel complies with the laws and regulations applicable to the vessel and that the vessel is otherwise satisfactory for the intended service. During inspection of the engine room cracked welds were identified in the vicinity of the port and starboard generator exhaust trunks. A crack was identified on the starboard side where the exhaust trunk penetrated the main deck causing water ingress. Vessel is to repair all cracks and perform non destructive testing on welds in the water tight envelope prior to departure.

The minimum capacity of the power fire pump must not be less than 60psi. Fire pump did not meet 60psi. Repair or replace fire pump to reach 60psi

All installed equipment must operate as designed. Fire detection panel on the bridge visual alarms were INOP. Repair to make operational.

During every drydock exam, the "leg to can" connection welds and the "can-pad" connection welds should be Non-destructively tested (NDT). Vessel did conduct NDT. Conduct NDT and provide report.

The master or person in charge shall ensure that each crane is operated and maintained in accordance with API 2D. Crane was being serviced and was inoperable. Provide report of crane repair.

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection and one at the end of the fuel line to stop fuel flow when servicing accessories. The shut off valve at the tank shall be operable from outside the tank compartment. During inspection it was discovered that the emergency fuel shut valves for the port and starboard fuel tanks were altered and moved to a secondary valve, approximately 5' from the valve installed at the fuel tanks. Vessel shall alter placement of emergency fuel shut off valves to be located at the closest valve to the fuel tanks. Vessel surpassed previous 14 day deadline issued on 835V from 09JUN2020 inspection. Vessel did not communicate needs for any additional time needed for correction of issued deficiency above. Additionally, vessel representative stated they will not fix until they "decide to pull out for service". The decision to not correct the issued deficiency shows the company is not proactively managing the potential fire hazard identified onboard. Vessel may not move from its present location or operate any machinery onboard until corrected. Vessel shall schedule a deficiency check inspection when corrections are made to ensure on site verification is completed to the satisfaction of the attending marine inspector. 46CFR182.20-40(b)(3)

The number and identification of the lifejackets must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Lifejackets that were located in accomodation spaces required labeling to identify thier location and number avaialble. 46 CFR 185(f)

Windows and other openings at the operating station must be of sufficient size and properly located to provide an adequate view for safe navigation in all operating conditions. Aft pilothouse window was damaged not allowing a clear view for navigation. 46 CFR 177.1030

Stove sea rails require suitable barriers to prevent accidental movement of cooking pots, must be installed on a cooking range. Newly installed stove did not contain sea rails. 46 CFR 184.220(e)

Fire Main overhead line in the stbd side engine room was not secured. Old brackets were not attached to the overhead deck. Reweld Fire Main brackets to overhead to secure Fire Main line.

Place double clamps on all coolings and hydraulic lines on both MDEs.

Place guards over rotating equipment on both MDEs.

Stbd side bilge pump was not operating. Properly operate s/s bilge pump.

General alarm and fire alarms were not correctly labeled. Fire alarms were labeled G.A. and vice versa. Correct labeling on all alarms.

Switchboard box missing plugs. Wires had been removed at some time in the past and the holes in the box for wires had not been plugged.

Provide 3rd party service report for satisfatory testing of bilge alarms

Demonsta=rate hydraulic system and components comply with 46 CFR 58.30 & 58.30 as required by NVIC 8-91, also that the system is "fail safe".

Provide approved KG curve in operations manual. Rep. stated that KG curve has been submitted to MSC and they are awaiting the results.

Demonstrate compliance with the 8th U.S. Coast Guard District's policy "Rescue Boat Requirements on Offshore Supply Vessels" dated 12NOV98 to the satisfaction of the attending Marine Inspector.

PROVIDE ON BOARD A COAST GUARD ISSUED STABILITY LETTER.

SUBMIT PLANS SHOWING GENERAL ARRANGEMENTS, INBOARD/OUTBOARD PROFILE AND A STRUCTURAL ANALYSIS OF ALL LEGS AND LEG TOWER SUPPORTS TO [name withheld] FOR APPROVAL.

SUBMIT AN OPERATING MANUAL MEETING THE APPLICABLE SECTIONS OF 46 CFR 109.121 TO [name withheld] FOR APPROVAL.

PROVIDE APPROVED STRUCTURAL AND ENGINEERING PLANS FOR SUBJECT VESSEL.

PROVIDE COAST GUARD APPROVED STABILITY LETTER/BOOKLET.

PROVIDE COAST GUARD APPROVED OPERATING MANUAL.

PROVIDE COPY OF CORRECTED CERTIFICATE OF DOCUMENTATION (DOC) TO REFLECT CHANGES IN ADMEASUREMENT.
