The company and the ship shall comply with the requirements of the ISM Code. Objective evidence discovered in an expanded ISM examination revealed that the vessel failed to fully implement the requirements of the SMS. Specifically, the company did not provide safe practices in ship operation and a safe working environment. This is made particularly evident by deficiencies #02, #03, #06, and #07. Additionally, the company failed to establish procedures that ensured the ship was maintained in conformity with the provisions of the relevant rules and regulations. This is made particularly evident by deficiencies #03, #04, #07, and #08. An external audit is required.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. All three ship's service generators have exposed rotating machinery of sufficient size to pose a serious personnel hazard.

Electrical installations shall be such that all electrical auxiliary services necessary for maintaining the ship in normal operational and habitable conditions will be ensured without recourse to the emergency source of electrical power. Due to water losses, the crew is manually adding water to the closed-loop cooling systems on all three ship's service generators to prevent loss of main power.

In order to ensure ready availability of the emergency source of electrical power, arrangements shall be made where necessary to disconnect automatically non-emergency circuits from the emergency switchboard to ensure that electrical power shall be available automatically to the emergency circuits. A pressure washer was hard-wired directly into the emergency generator switchboard to enable its temporary use.

Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. Special consideration shall be given to the malfunctioning of, among other systems, the fuel oil supply systems for boilers or engines. There is a leak in the piping to the fuel oil transfer pump #2, significant enough that the crew stopped using the pump for safety reasons, leaving only a single fuel oil supply pump available.

The function of fixed fire detection systems shall be periodically tested to the satisfaction of the Administration by means of equipment producing hot air at the appropriate temperature, or smoke or aerosol particles having the appropriate range of density or particle size, or other phenomena associated with incipient fires to which the detector is designed to respond. No intended fire detection testing method was on board, and the vessel’s crew was conducting periodic tests of the fire detection system using exhaled cigarette smoke, with no target temperature, density, or particle range, or means of measuring those parameters.
