Each seperate circuit shall be protected against short circuit and against overload. 02 overloaded circuits identified on bridge. 74SOLAS2004 II-1 45.6.1

All life-saving appiances and arrangements shall comply with the applicable requirements of the code. All lifesaving appliances prescribed in this part shall... Be fitted with retro-reflective material where it will assist in detection and in accordance with the recommendations of the organization. Life buoys on each bridge wing and lifeboat were weathered. 74 SOLAS 2004, III 34 LSA 1.2.2.7

Escape routes shall be maintained in a safe condition, clear of obstacles. Refrigerator door, leading into vegetable room, has an inoperable handle, impeding means of escape. 74SOLAS2004 II-2 13.1.2

FIRE-FIGHTING SYSTEMS AND APPLIANCES SHALL BE KEPT IN GOOD WORKING ORDER AND READILY AVAILABLE FOR IMMEDIATE USE. DURING THE TESTING OF THE SMOKE DETECTOR ABOVE HFO SEPERATOR NO.1, PSCO OBSERVED THE DETECTOR ACTIVATE BUT WAS UNABLE TO BE SILENCED OR ACKOWLEDGE, RENDERING THE REMAINING 5 SMOKE DETECTORS IN THAT ZONE INCAPABLE OF BEING TESTED. SOLAS74(04)II-2/14.2.1.2

THE FOLLOWING FIRE PROTECTION SYSTEMS SHALL BE KEPT IN GOOD WORKING ORDER SO AS TO ENSURE THEIR REQUIRED PERFORMANCE IF A FIRE OCCURS: STRUCTURAL FIRE PROTECTION INCLUDING FIRE-RESISTING DIVISIONS, AND PROTECTION OF OPENINGS AND PENETRATIONS IN THESE DIVISIONS. PSCO OBSERVED CLASS A BOUNDARY WITH INSULATION IN DISREPAIR AND A CABLE RUN INADEQUATLEY SEALED IN DRY STORES. SOLAS74(04)II-2/14.2.1.1

DURING PREPARATION AND LAUNCHING THE SURVIVALCRAFT, ITS LAUNCHING APPLIANCE, AND THE AREA OF WATER INTO WHICH IT IS TO BE LAUNCHED SHALL BE ADEQUATELY ILLUMINATED BY LIGHTING SUPPLIED FROM THE EMERGENCY SOURCE OF ELECTRICAL POWER REQUIRED BY II-1/42 OR II-1/43 AS APPROPRAITE. PSCO OBSERVED NO LIGHTING ARRANGEMENT AT THE FORWARD LIFERAFT CAPABLE OF LIGHTING THE LAUNCHING APPLIANCE AND THE AREA OF WATER WHICH IT IS TO BE LAUNCHED. SOLAS74(20)III/16.7

OIL RECORD BOOK SHALL BE IN THE FORM SPECIFIED IN APPENDIX III TO THIS ANNEX. APPENDIX III REFERS TO MEPC.1/CIRC.736/REV.2 ANNEX PAGE 1: IF A WRONG ENTRY HAS BEEN RECORDED IN THE OIL RECORD BOOK (ORB) IT SHOULD IMMEDIATELY BE STRUCK THROUGH WITH A SINGLE LINE IN SUCH A WAY THAT THE WRONG ENTRY IS STILL LEGIBLE. PSCO OBSERVED ENTRIES IN THE ORB WHITED OUT. MARPOL ANNEX 1/17.1

Fire-fighting systems and appliances shall be kept in good order and readily available for immediate use. PSCO observed releasing control for CO2 cylinder to engine room was not readily available for immediate use due to being zip-tied in closed position. Pneumatic piping is not intact due to missing nozzle caps. Flexible discharge hose and fitting was not attached/secured to CO2 cylinder. SOLAS 74 (04)II-2/14.2.1.2

Maintenance testing and inspection shall be carried out based on the guidelines developed by the organization, and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. On board maintenance inspections are not being carried out in accordance with the ship's maintenance plan, which shall include the minimum requirements of sections 4 through 10 of MSC.1/CIRC.1432 Annex. Existing maintenance plan did not include minimum requirements along with objective evidence that required maintenance, testing and inspections have been carried out to those standards. SOLAS 74 (04) II-2/14.2.2.1

A fixed fire detection and fire alarm system and a sample extraction smoke detection system required in this regulation and other regulations in this part shall be of an approved type and comply with the Fire Safety Systems Code. Suitable testing and component spares for testing and maintenance shall be provided. Detectors shall be periodically tested using equipment suitable for the types of fires to which the detector is designed to respond. Chief Engineer stated the vessel does not have the proper equipment to test the flame detectors properly. SOALS 74 (04) II-2/2.2.2 FSS Code 9.2.5.2

The maintenance plan shall include those fire protection systems and firefighting systems and appliances in accordance with regulation 14.2.2.3. PSCO discovered maintenance plan does not address installed water mist system or EEBDs maintenance elements. SOLAS 74 (20) II-2/14.2.2.3

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with additional requirements which may be established by the company. Due to the deficiencies of a technical and/or operational nature identified below which individually or collectively provides objective evidence of the ISM code. An external audit shall be conducted by the Flag Administration before the vessel will be released from its detention to determine whether the vessel is operating in compliance with the ISM code. A copy of the completed external audit shall be provided to this office for review prior to departure from the port of Pensacola. (74SOLAS (20) IX/3.1 ISM Code Part A/10.1)

Before the ship leaves port and at all times during the voyage all life saving appliances shall be in working order and ready for immediate use. PSCO observed rescue boat engine unable to start. (74SOLAS (20) III/20.2)

The electrical power available shall be sufficient to supply all those services that are essential for safety in an emergency. PSCO observed during the testing of the emergency generator, that the emergency generator was not providing sufficient voltage or Hz to supply the emergency switchboard. (74SOLAS (04) II-1/43.2)

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. PSCO observed that fire boundary doors, in vessel stairway enclosure, were unable to close and latch properly. (74SOLAS (04) II-2/14.2.1.1)

Fire protection systems and fire-fighting systems and appliances shall be maintained and ready for use. PSCO observed washers for international shore connection missing. (74SOLAS (04) II-2/14.1.1 FSS Code 2.2.2)

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed stuffing tubes on the service diesel generator were loose and not maintaining cable strain. PSCO also observed light switch in hospital space not secured properly. (74SOLAS (04) II-1/45.1.3)

Steering gear compartment arrangements shall include handrails and gratings or other non-slip surfaces to ensure suitable working conditions in the event of hydraulic fluid leakage. PSCO observed no gratings, mats, or other means of providing a non-slip surface in the steering gear room. (74SOLAS (04) II-1/29.13.2)

Simple operating instructions with a block diagram showing the change over procedures for remote steering gear control systems and steering gear power units shall be permanently displayed on the navigation bridge and in the steering compartment. PSCO observed no block diagram posted in steering gear room. (74SOLAS (20) V/26.3.1)

The bunker delivery note shall be accompanied by a representative sample of the fuel oil delivered taking into account the guidelines developed by the organization. PSCO requested fuel oil samples but the vessel could not locate the two most recent bunkering samples. (MARPOL VI/18.8.1)

In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed, and maintained in compliance with the mechanical requirements of a classification society which is recognized by the Administration in accordance with the provisions of regulation XI/1, or with the applicable national standards of the Administration which provide an equivalent level of safety. PSCO observed acetylene and oxygen cylinders not secured in rack and missing caps. (74SOLAS (04) II-1/3-1)

The operator of a vessel with a capacity of 250 or more barrels of oil, hazardous material, or liquefied gas as regulated in Table 4 of 46 CFR part 154 shall provide transfer procedures that meet the requirements of this part and part 156 of this chapter. Vessel was unable to provide transfer procedures that met the aforementioned requirements to the PSCO. (33 CFR 155.720)
