An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Port and starboard main engines do not have protection installed on the pulley systems.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. An unauthorized modification was made to the vessel’s electrical system and watertight boundaries. New cabling had been installed from starboard distribution panel, extending forward through multiple designed boundaries, and terminating in the bar area. Cabling was not properly secured, penetrated at least one watertight bulkhead, and served an unknown purpose. Submit detailed plans for the modification, addressing safety concerns, to the OCMI for review, approval, and inspection. 46 CFR 176.700(a)

A pre-engineered fixed gas fire extinguishing system must: automatically shut down all engines that draw intake air from within the protected space and have a means to reset devices used to automatically shut down engines. Starboard MDE did not shutdown when fireboy circuit was pulled and port MDE was unable to restart when override switch at operating station was engaged. Provide proper shutdowns and override restart functions to Starboard and port pre-engineered fixed gas fire extinguishing systems. 46 CFR 181.420(a)(3)

A visual and audible alarm must be provided at the operating station to indicate a high water level in a machinery space. Port engine room high level bilge alarm visual and audible indication is not operational. Provide functioning visual and audible alarms. 46 CFR 182.530(a)(2)

A penetration in a watertight bulkhead must be made watertight and machinery spaces must be made vapor tight to prevent passage of vapors, smoke and flame. The fire caulk in the cable penetration of the port forward engine room bulkhead had been damaged during possible cable removal and not been properly restored to an acceptable condition. 46 CFR 179.320 & 46 CFR 177.405(c)

Piping of which the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. Exhaust lagging on the port main engine was found not fully installed, leaving hot surfaces exposed at the crews point of entry. 46 CFR 177.970

Spanner wrenches must be provided where a 1.5 inch diameter fire hose is required. Provide Spanner wrench at fire station. 46 CFR 181.310(d)

The design, materials, and construction of masts, posts, yards, booms, bowsprits, and standing rigging on a sailing vessel must be suitable for the intended service. Following the annual third party rig survey conducted on 15APR2026, numerous cracks were observed in the swage fittings throughout the rigging aloft, including the diamond wires and the screecher stay. The extent and distribution of these defects indicate premature failure of the fittings themselves. The exact cause is currently unknown. As recommended by 3rd party surveyor [name withheld], vessel shall not be permitted to operate under sail until the rigging has been replaced. The rigging appears to be stable for operation under motor power only. Aloft inspections shall be increased to a weekly frequency until the rigging is replaced and shall be thoroughly documented in the rigging log or digitally recorded with time-stamped photographs or both. All standing rigging shall be replaced all within 90 days. Survey Report to follow. 46 CFR 177.330

Each vessel certificated to operate on a limited coastwise route within three miles of land in warm water must have on board a FCC Type Accepted Category 1 406 MHz EPIRB, installed to automatically float free and activate, and not be required to carry survival craft. Vessel removed life rafts for annual servicing and shall be limited to operations carrying passengers within three miles of land until life rafts are verified as serviced and installed on vessel. 46 CFR 180.205(e)(3)

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Wiring for the water pump for the vessel's water slide in the port steering gear compartment was improperly spliced together. Use suitable connection to connect conductors to the satisfaction of the attending CG marine inspector. 46 CFR 183.200 (b)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Marine inspector found electrical tape covering conductor connection on wiring for HVAC system in the galley, port side cabinet. Remove electrical tape for inspection by CG marine inspector. 46 CFR 176.840

The owner or managing operator of a vessel shall ensure that the vessel complies with the laws and regulations applicable to the vessel and that the vessel is otherwise satisfactory for the intended service. The initial inspection may include an inspection of the following items: Certificates and operating manuals, including certificates issued by the FCC. FCC Safety Radio Certificate not issued by FCC licensed technician for radio installation. Provide inspection and valid FCC Safety Radio Certificate. 46 CFR 176.402

The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by Table 181.500(b). Missing 01 portable extinguisher. 46 CFR 181.500(a)

A fire pump must be capable of both remote operation from the operating station and local operations at the pump. Fire pump did not have remote capability from the operating station. Provide means of operation of fire pump from operating station. 46 CFR 181.300(e)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. Vessel did not have the number of passengers allowed posted on port or starboard aft stairwells. Provide marking for both stairwells. 46 CFR 185.602 (g)
