Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by table 1 to § 181.500(b) and other provisions of this section. Vessel was missing one 40 B.C. outside engine compartment. COI did only show one 10 B.C. required to be onboard but this is incorrect for vessel operation. 46 CFR 181.500(a)

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Vessel's life ring buoy was missing sections of type II retroreflective tape on buoy. 46 CFR 185.604(i)

An adult life jacket must be provided for each person carried on board a vessel. Vessel had 18 life jackets instead of the 19 listed on the COI. Attain equal number of life jackets listed on the COI. Passengers are limited to 18 until on adult lifejacket is obtained on vessel. 46 CFR 180.71 (a).

A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Medical certificates must be issued and will remain current for the following periods of time, unless otherwise noted on the certificate. (1) Two years for individuals serving on vessels to which STCW applies unless the mariner is under the age of 18, which case the maximum period of validity will be 1 year. (2) Five years for all other mariners. Several operators had expired medical certificates. Operators need to have valid medical certificates to operate launches. 46CFR15.401(c)(1)(2)

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Crew didn't have records for drills. Drills need to be logged for fire, man overboard, and abandon ship. 46CFR185.524(d)(1)(2)
