PSCO observed vessel did not conduct or have record of conducting a risk assessment prior to sending crew forward on deck during heavy weather IAW SMS requirement for heavy weather work permit and no permit was issued. Crew stated they went forward to BSN store in 30 degree rolls. Crew had no knowledge of heavy weather work permit. 74 SOLAS (20) IX/3 ISM CODE PART A/7
