Gas detection equipment shall be designed, installed and tested in accordancewith a recognized standard. PSCO observed two gas detection alarms at GDS3-3 - LNG Tank Vent Mast measuring 100+% LEL and GDS3-4 - FGSS Vent Mast measuring 4$ LEL. Additionally, when ask to test fixed as detection point at FWD Port Side bunker station. The measurement went immediately to 100% LEL. It was determined that the Electrican was using 60% by vol methane span gas. The crew was able to locate the approved 50% LEL span gas to again. The same point was tested and results measured 58% LEL and finished at 100% LEL. A third test was conducted at AFT Port Bunker station using the 50% LEL, which only measured 20% LEL.

Resolution: gas detector found to be defective, replaced with spare one by crew. It
has been tested again and appeared same alarm 100% LEL. LNG tank vent mast was purged with
nitrogen, then LEL % reduces. Presently the gas detection system is normal

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. PSCO observed the Electrian using 60%% by vol methane span gas to test fixed gas detection. As noted by 10 Calibration Instruction for Salwico ST200 and Salwico ST 600EX 50% LEL gas of methane is to be used.

The Master or person in charge of a vessel equipped with ballast tanks that operates in the Waters of the United States must employ one of the following ballast water management methods: install and operate a ballast water management system that has been approved by the Coast Guard. Following installation, the Master or person in charge of the vessel subject to this subpart must properly maintain the BWMS in accordance with all manufacturer specifications. It was reported to the PSCO that the positioner valve to the UV reactor was inoperable, which resulted in the Ballast Water Treatment System being inoperable.
