A pilot ladder shall be certified by the manufacturer as complying with the regulation or with an international standard acceptable to the organization PSCO observed the absence of a valid certificate for the port side pilot ladder. PSCO observed discrepancies between the certificate and physical attributes of the starboard side pilot ladder. The certificate and physical attributes of the starboard side pilot ladder. The certificate describes the ladder as having 33 wooden steps, 5 spreaders and an overall length of 20 meters, whereas the actual ladder consists of 44 wooden steps, 6 spreaders, and was 18 meters in overall length. PSCO believers this to be a fraudulent ladder and certificate. 74 SOLAS (20) V/23.2.3

The normal equipment of every lifeboat shall consist of a food ration totaling not less than 10,000 kJ for each person the lifeboat is permitted to accommodate. These rations shall be kept in airtight packaging. PSCO observed 17 rations out of the 30 in the lifeboat did not have an airtight seal. 74 SOLAS (20) III/34 LSA 4.4.8.12

Due to the objective evidence in the above deficiencies the coast guard questions the adequacy and overall implementation of the vessels SMS under the ISM code. An internal audit is required within 30 days. Provide a copy of audit report to USCG. 74 SOLAS (20) IX/3.1
