Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. Multiple lifejackets and immersion suits did not have lights and whistles attached as required by the LSA Code.

Resolution: Vessel master provided pictures of lifejackets and immersion suits with lights and whistles attached as well as an email chain from the ship to the company and Class acknowledging the correction.
