MI observed a fuel leak from the Stbd Main Engine Fuel Pump (1 drop every 10 seconds). Repair to the satisfaction of the Marine Inspector.

MI observed Steering Hydraulic components not secured by permanent means, causing hydraulic lines/valves to move while system is operating. Repair to the satisfaction of the Marine Inspector.

Decks and bulkheads designed to be watertight or weather tight must be maintained in that condition. MI observed a electrical pipe cracked leading to the pilot house resulting in water intrusion to electrical components. Repair to the satisfaction of the Marine Inspector.

A fire-detection system is installed, tested, and maintained in accordance with the manufacturer's design manual. Marine Inspector observed the fire-detection panel showing a fault alarm. Repair to the satisfaction of the MI. 46 CFR 142.330(a)(2)

A fire-detection system is installed, tested, and maintained in accordance with the manufacturer's design manual. Marine Inspector observed no testing equipment to ensure functionality of fire detection in the engine compartment. Repair to the satisfaction of the MI. 46 CFR 142.330(a)(2)

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention to the Port and Starboard Main Engine’s by providing a low lubricating oil pressure and high cooling water temperature alarms. Marine Inspector was not able to confirm working alarm set points on Port and Starboard Main Engine’s. Vessel requires confirmation of working alarm set points in the presence of a USCG Marine Inspector. 46 CFR 143.230

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention to the Port and Starboard Generators by providing a low lubricating oil pressure and high cooling water temperature alarms. Marine Inspector was not able to confirm working alarm set points on Port and Starboard Generators. Vessel requires confirmation of working alarm set points in the presence of a USCG Marine Inspector. 46 CFR 143.230

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention to the Hydraulic Steering System by providing a low fluid level alarm. Marine Inspector was not able to confirm the low level hydraulic steering alarm. Vessel requires confirmation of low fluid level alarm in the presence of a USCG Marine Inspector. 46 CFR 143.230

A vessel of 12 meters or more in length shall be provided with a whistle. Maine Inspector observed vessel's installed whistle not functioning. Repair to the satisfaction of the MI. 33 CFR 83.33 (a)

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch (PSI) maximum allowable working pressure (MAWP) must be equipped with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP, as established by the manufacturer, by more than 10 percent. Marine Inspector observed no pressure relief valve tag or documentation attesting to relieving pressure PSI. Provide relief valve documentation and/or valve tag approving relief pressure to the MI. 46 CFR 143.300 (a)

Individual circuit breakers on switchboards and distribution panels must be labeled with a description of the loads they serve. Marine Inspector observed circuit breaker in engine compartment not labeled with specific equipment each breaker energizes. Repair to the satisfaction of the MI. 46 CFR 143.400 (d)

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. Acceptable methods of securing the device include: Padlocking the seacock in the closed position. Marine Inspector sewage discharge valve unlocked in the closed position. Repair to the satisfaction of the MI. 33 CFR 159.5 (b)(2)

Every towing vessel equipped with an inflatable survival craft must have, in conspicuous places near each inflatable survival craft, approved placards or other posted instructions for launching and inflating inflatable survival craft. Marine Inspector observed no posted instructions for launching/inflating the vessels inflatable survival craft near the stowage location. Provide placard's or posted instructions to the satisfaction of the MI. 46 CFR 141.320

All pressure vessels must have the maximum allowable working pressure (MAWP) indicated by a stamp, nameplate, or other means visible to the crew. Marine Inspector observe pressure vessel without MAWP indicated on tank. Repair to the satisfaction of the MI. 46 CFR 143.300 (c)

A towing vessel may not be operated without having onboard a valid COI issued by the Coast Guard as required by 136.202. The vessel must obtain a valid COI prior to conducting cargo operations (pushing/pulling). 46 CFR 136.200

The owner or managing operator of an existing vessel not operating under a stability document must be able to show at least one of the following: 1. The vessel's operation or a history of satisfactory service does not cause the stability of the vessel to be questioned by either the Coast Guard or a TPO engaged to perform an audit or survey. 2. The vessel performs successfully on operational tests to determine whether the vessel has adequate stability and handling characteristics. 3. The vessel has a satisfactory stability assessment by means of giving due consideration to each item that impacts a vessel's stability characteristics which include, but are not limited to, the form, arrangement, construction, number of decks, route, and operating restrictions of the vessel. MI observed that a COI on board was endorsed for a coastwise route; however, there is no evidence of the above requirements being conducted. Until stability is proven vessel is limited to a lakes, bays, and sounds route. 46 CFR 144.300(b)(1)-(3)

A fixed fire pump must be capable of delivering water simultaneously from the two highest hydrants, or from both branches of the fitting if the highest hydrant has a Siamese fitting, at a pitot-tube pressure of at least 344 kilopascals (kPa) (50 pounds per square inch (psi)), and a flow rate of at least 300 liters per minute (lpm) (80 gallons per minute (gpm). MI observed gauge indicating pressure was inoperable, and observed pressure was questionable to the standard. 46 CFR 142.325(a)(1)

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. MI observed both aft hatches leading into the engine room had wasted gaskets, and allowed water to ingress when tested with hose. In addition the stbd side hatch was missing it's handle. 46 CFR 140.610(b)

Remote fuel shutoffs must be operable from a safe place outside the space where the valve is installed. MI observed that multiple remote fuel shut offs were inoperable. 46 CFR 143.260(d)

The fire detection system must be installed, tested, and maintained IAW the manufacturers design manual. The fire detection panel had ground faults and was unable to be tested. 46 CFR 142.330

Electrical components that pose an electrical hazard must be in an enclosure. Dead end wires were found in the engine room above the battery charger. 46 CFR 143.400

Exposed hazards such as rotating machinery must be protected by a cover, guard, or rail. The damper on the forward end of both engines need a guard installed. Stbd engine needs a guard over the alternator pulleys. 46 CFR 144.820

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. MI identified an accumulation of oil in engine room bilge well. 46 CFR 142.220 (a)

Each towing vessel must be fitted with a general alarm that has installed in the engine room a supplemental flashing red light with a sign that reads Attention General Alarm – When Alarm Sounds Go to Your Station. Vessel did not have the flashing red light and sign installed in the engine room. 46 CFR 143.235

When the towing vessel is underway the EPIRB must be stowed in its float free bracket in a manner so it will float free. The hydrostatic release on the EPIRB was past its expiration date. 46 CFR 141.380

Each towing vessel operating on oceans, coastwise, or Great Lakes routes must have a means to take blood pressure readings, splint broken bones, and apply large bandages for serious wounds. Marine inspector observed that the vessel did not have a means of obtaining blood pressure. 46 CFR 140.435

Each vessel with a marine sanitation device must have manufacturers instructions containing directions for the type and quantity of the chemicals that are required to operate the device, including instructions on the proper handling, storage, and use use of these chemicals. Marine inspector observed that the vessel did not have the instructions onboard. 33 CFR 159.57(a)(5)

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the EPA under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. Marine inspector observed sewage valve for overboard discharge was unlocked. 33 CFR 159.7(b)

The towing vessel record must have a record of the navigational assessment and the navigational training required by 46 CFR 140.635 and 140.645 respectively. Marine inspector observed that the vessel was not logging any of the navigation assessments or navigation training. 46 CFR 140.915

Each lifeline on a lifebuoy must have a breaking strength of at least 5 kilonewtons and be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. MI identified multiple lifebuoys with deteriorated lifelines and insufficient retro-reflective material. 46 CFR 141.350(c)(1)(v)

Pressure vessels must be externally examined annually. Relief valves must be tested in accordance with § 143.245. All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. MI identified pressure vessel for compressed air with no visible indication of the MAWP for the pressure vessel and no evidence that the relief valve had been tested. 46 CFR 143.300

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. MI identified ventilation openings for engine room with no means of closing off ventilation. 46 CFR 144.605

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. The vessel has a COI anniversary date of 15Apr. The vessel was required to have an annual inspection no later than 15Jul23, and the vessel has not had an annual inspection for this year. 46 CFR 137.200

Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. COD expired 30 November 2021.

ll portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months. Inspection was past due in June, 2022.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. No smoke detector in berthing off galley/messdeck.

ifejackets must have the following attachments and fittings: Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket and each lifejacket must have a whistle firmly secured by a cord to the lifejacket. Missing both light and whistle on all lifejackets.

At least one lifebuoy must have a lifeline, secured around the body of the lifebuoy. If more than one lifebuoy is carried, at least one must not have a lifeline attached. All lifebuoys had lifelines attached.

Except as noted in paragraph (c) of this section, stations in the maritime service must be licensed by the FCC either individually or by fleet. Bridge to Bridge license was expired.

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. The navigation assessment must incorporate the requirements of pilothouse resource management of § 140.640, assess operational risks, and anticipate and manage workload demands. Navigational assessments were not being conducted.

Each towing vessel must be fitted with a general alarm that: Has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: “Attention General Alarm - When Alarm Sounds or Flashes Go to Your Station”. No signage in engine room for general alarm.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. First aid kits were expired.

The drydock examination must be conducted while the vessel is hauled out of the water. The following worklist items a) through i) shall be addressed and approved by a Marine Inspector before the vessel is refloated. The remaining worklist items shall be completed before receiving credit for drydock. a) Replace starboard aft rudder post flange bolts b) Determine hull thickness at starboard shaft tube and bottom plating in vicinity of propellers c) Replace both propellers and water-lubricated bearings d) Insert hull plating on starboard side aft in line with engine room door e) Insert hull plating on starboard side in vicinity of forward compartment bulkhead f) Insert hull plating on port side turn of the bilge outside of keel cooler g) Weld wash in the following areas: port and starboard side turn of the bilge amidships and forward, three longitudinal butt welds on forward rake hull plating h) Wasted flange on starboard shaft packing gland i) Starboard fuel tank; broken welds vertical stiffener and transverse frame (2nd from aft bulkhead) j) Two stanchions supporting deck grating over steering gear damaged k) Two vents wasted on port side in vicinity of the lazarette scuttles l) Recoat potable water tanks m) Unseal two deck scuttles for lazarette access n) Wasted stiffener on port amidships bulkwark in vicinity of line roller o) Descale for further inspection: port and starboard forward bitts on outboard sides, starboard aft bitt on outboard side, forward main deck in vicinity of centerline bitt and starboard side winch p) Remove forward compartment decking for inspection of internal structure of vessel q) Insert required on starboard push knee in forward compartment r) Wasted bulwark stiffeners on starboard side amidships and starboard aft in vicinity of lazarette scuttle s) Wasted rudder stop chains t) Proper tray for starboard forward generator battery u) Improper sealing of bulkhead penetrations in fiddley

ELECTRICAL CONNECTIONS MUST BE SUITABLY INSTALLED TO PREVENT THEM FROM COMING LOOSE THROUGH VIBRATION OR ACCIDENTAL CONTACT. THE PORT AFT BATTERY HAD TEMPORARY CLAMPS. INSTALL PERMANENT ELECTRICAL CONNECTIONS.

EACH WIRE MUST BE INSTALLED IN A MANNER TO PREVENT CONTACT WITH PERSONNEL, MECHANICAL HAZARDS, AND LEAKING FLUIDS. MI OBSERVED WIRES UNDER STEERING CONSOLE EXPOSED AND WIRES PROTRUDING FROM MAIN ENGINE GAUGES EXPOSED. ENSURE THESE WIRE ARE PROPERLY COVERED.

TOWING VESSELS MUST CARRY 3 DAY AND 3 NIGHT VISUAL DISTRESS SIGNALS WHEN OPERATING ON LAKES, BAYS, SOUNDS, AND RIVERS WITHIN 3 NAUTICAL MILES. VESSEL HAD 3 NIGHT AND ONE DAY DISTRESS SIGNAL. PROVIDE APPROPRIATE AMOUNT OF DISTRESS SIGNALS.

THE MAIN ENGINES MUST BE EQUIPPED WITH RPM GUAGES. MI DID NOT OBSERVE ANY RPM GAUGES AT THE MACHINERY SPACE FOR MAIN ENGINE RPM. INSTALL MAIN ENGINE RPM GUAGE IN THE MACHINERY SPACE.

EACH REMOTE FULE SHUTOFF VALVE CONTROL MUST BE MARKED INDICATING THE PURPOSE. NO INDICATIONS WERE PRESENT. PROVIDE INDICATIONS FOR EACH VALVE AT THE REMOTE CONTROL.

FIRE DETECTION SYSTEM PANEL THAT INCLUDES LABELS FOR EACH AREA. FIRE DETECTION PANEL DID NOT INDICATE EACH AREA IT PROTECTED. PROVIDE IDENTIFICATION FOR EACH AREA AS APPROPRIATE.

ALL PORTABLE FIRE EXTINGUISHER, SEMI-PORTABLE FIRE EXTINGUISHERS MUST BE INSPECTED AT LEAST ONCE EVERY 12 MONTHS AND FREQUENCY SPECIFIED BY NFPA 10. NO DOCUMENTATION OF MAINTENANCE WAS PROVIDED. PROVIDE MAINTENANCE FOR ANNUAL AND OTHER INSPECTIONS.

THE COAST GUARD WILL ISSUE A MEDICAL CERTIFICATE TO MARINER MEETING MEDICAL AND PHYSICAL STANDARDS FOR MERCHANT MARINERS AND WILL BE ISSUED BASED UPON ENDORSEMENT. MARINER UNABLE TO PROVIDE MEDICAL CERTIFCATE. PROVIDE PROOF OF MEDICAL CERTIFICATE.

Each towing vessel must have a fire-detection system that is installed , tested and maintained in accordance with the manufacturer's design manual. Vessel installed a battery cutoff switch for on fire-detection panel's secondary source of power. Uninstall battery cutoff switch.

Propulsion machinery must be maintained to ensure proper operation. Port shaft packing gland is leaking at approximately 4 drops per second while moored. Repair leak.

Each vessel required to have a Certificate of Inspection is subject to the annual inspection fee listed in Table 2.10-101 for its vessel category. USCG records show an outstanding balance of $13.06 for the annual user fee. Vessel must settle any balance for annual fees or late penalties with FINCEN Customer Service.

All fire-detection systems must be inspected or tested at least once every 12 months. The fire-detection system has not been tested or inspected in over a year. Inspect fire-detection system and provide proof.

Each towing vessel must be arranged so that any essential system identified by the cognizant OCMI can be energized from two independent sources. The motor controller for both the fire pump and the bilge pump were inoperable. Repair both motor controls.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and must be automatic. The emergency lighting did not turn on when the vessel went from shore power to ships power. Repair emergency lighting.

Electrical components that pose an electrical hazard must be in an enclosure. 1. The port/stbd shaft packing salt water containment box bilge pumps are plugged into an outlet that is spliced into an extension cord, zipped tied to a pipe. Install proper outlet that is powered from a distribution panel. 2. There are 5 dead ended wires under the forward bridge console. Determine what these wires are for and either connect them properly, remove them, or terminate them into a junction box.

Each integral fuel tank must have a vent fitted with a 30 - by - 30 mesh corrosion resistant flame screen. The four fuel oil vents do not have 30 - by - 30 mesh flame screens. Install proper flame resistant mesh screen.

A fixed fire pump must be capable of delivering water at a pitot-tube pressure of at least 50 pounds per square inch (psi). The gauge of the fire hose was only reading 36 psi. Determine if the hose pressure or the gauge is not working properly and repair or replace in kind.

A towing vessel must carry adequate and up to date nautical publications. Vessel did not have a Coast Pilot on board. Obtain current edition of Coast Pilot.

There must be an installed or portable bilge pump for emergency dewatering. Bilge suction not operational. Demonstrate operation of bilge pump.

The owner, managing operator, or master of each towing vessel must maintain the required navigational-safety equipment in a fully-functioning, operational condition. Vessel does not have day shapes. Provide day shapes.
