The use of cylindrical gauge glasses is prohibited. The Administration may permit the use of oil-level gauges with flat glasses and self closing valves between the gauges and fuel tanks. PSCO received email stating that there was an issue with the starboard engine control lever. Causing the starboard propeller's pitch to not increased more than 42 pitch, hindering vessel's ability to maneuver.

On completion of loading the ship and prior to its departure, the master shall determine the ship’s trim and stability and also ascertain that the ship is upright and in compliance with stability criteria in relevant regulations. The determination of the ship’s stability shall always be made by calculation or by ensuring that the ship is loaded to one of the pre-calculated loading conditions within the approved stability information. The vessel’s lightship value changed from 1970.938MT to 2163.661 MT, but the loading computer does not incorporate this new lightship value and the crew is incapable of modifying it. The vessel departed from the Netherlands and arrived into Providence, RI without completing stability calculations.

Ballast water record book for WBT #7 indicates that there is 20m^3 of ballast in the tank, but radar gauging indicates 8m^3. Manual sounding of the tank indicates the actual value is closer to 20^m3. The radar gauge is malfunctioning and the crew indicated that a similar problem exists in other WB tanks. RO to sound and verify that all water ballast tank radar gauges are functioning and reporting accurate values to the actual sounding levels.
