Fire plans and booklets shall be kept up to date, any alterations being recorded theron as soon as practicable. The fire control plan did not correlate with the arrangements on board the vessel. SOLAS (74) II-2 Reg 4

Each panel board switching unit must have a circuit directory that has the circuit designation of each circuit. The switchboard panel on bridge was not updated to the current circuit arrangement. 46CFR 111.40-11

Ships shall carry a line throwing appliance of an approved type. The appliance shall include not less than four projectiles and four lines. Only 1 charge was observed on board the vessel at time of inspection. SOLAS (74) III Reg 23

Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must include fire detection and extinguishing, flooding safety, propulsion, maneuvering, electric power generation and distribution, and emergency internal communications. The testing procedure to verify back up power was in use for the fire detection system needs to be updated to correspond with system. 46CFR 61.40-6

Each discharge to sea or to a reception facility shall be promptly recorded in the garbage record book and the entry for each discharge or incineration shall include the date and time, position of the ship, category of the garbage and the estimated amounted discharged or incinerated. The garbage record book entries did not correlate with the garbage receipts from the reception facility that received the garbage on 01JUL2024. Approx. 2 cubic meters of garbage was unaccounted for in the garbage record book and the date of discharge was different from what the garbage receipt indicated. MARPOL (2017 cons) Annex V Reg 10.3.1.2

Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the Oil Record Book Part I, so that all entries in the book appropriate to that operation are completed. Vessel did not record accurate entries.

Manual alternate control systems must - be readily accessible and operable. Emergency stop on STBD Aux Gen inoperable.

All vessels except unmanned barges must be provided with a satisfactory bilge pumping plant capable of pumping from and draining any watertight compartment except for ballast, oil, and water tanks. The bilge pumping system must be capable of operation under all practicable conditions after a casualty whether the ship is upright or listed. Bilge pump in E/R leaking at seal when operated. Repair to satisfactory condition.

In a space described in § 113.25-9(a), where the general emergency alarm signal cannot be heard over the background noise, there must be a red-flashing light or rotating beacon, in addition to the general emergency alarm signal, that: Is activated whenever the general emergency alarm signal in the space are activated. Red-flashing light or rotating beacon not activated when general emergency alarm is activated.

The door of each panel board enclosure that is accessible to any passenger must have a locking device. 02 panel boards in E/R utilizing unauthorized securing devices.

Each pressure vessel, other than one exempted by this section, must be subjected to a hydrostatic test at a pressure of 1 1/4 times the maximum allowable working pressure twice within any five-year period, except that no more than three years may elapse between any test and its immediate predecessor. Unable to satisfactorily examine pressure vessels (02) internally.

All ships of 150GT and upwards shall be fitted with a daylight signaling lamp, or other means, to communicate by light during the day. Vessel does not have a daytime signaling lamp. SOLAS 74 (2020) CH. 5 Reg 2.2.2

Each vessel must have suitable pilot boarding equipment available for use on each side of the vessel with a pilot ladder approved under 46 CFR 163.003. The vessel did not have suitable pilot boarding equipment or a pilot ladder of an approved type.

Every ship to which Chapter I applies shall be issued a Continuous Synopsis Record. The vessel's CSR lists them as having a Class Society when they are not a classed vessel. SOLAS 74 (2020) CH XI-I, Reg 5

Complete but simple instructions for the operations of systems must be located in a conspicuous place. The instructions for the machinery space CO2 system are incorrectly written.

Every inflatable liferaft shall be serviced at intervals not exceeding 12 months. Annual servicing was improperly marked on life raft. Servicing label extends beyond 12 months since service date. SOLAS 74 (2020) CH III, Reg 8.1.1

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work. The vessel does not have an up-to-date general arrangement plan, subdivision and stability plan, fire control plan, marine engineering plan, electrical engineering plan, embarkation plan, or navigation bridge visibility plan.

Update automation test procedures to capture the additional required alarm and safeties for approval.

Detectors, manual alarm stations, control panels, alarms, and other notifying devices must be of approved types. Vessel's detectors, manual alarm stations, control panels, and alarms are not approved by MSC.

Each circuit that supplies a device on a switchboard must have overcurrent protection. Amperage mismatch on aft distribution panel for aft flood light.

Each opening through which a conductor enters must be closed. Overhead junction box has conductors protruding out of enclosure.

Each outlet or junction box must be securely attached to its mounting and be affixed so as to maintain its designated degree of protection. Junction box found to be hanging above port ladder well in engine space.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Fuel pressure sensor on #1 SSDG needs to be repaired in accordance with manufacturer's specifications.

Oil filtering equipment shall be provided with alarm arrangements to indicate oil content does not exceed 15 parts per million (PPM) when this level cannot be maintained. During testing of oil filtering equipment, only an indicating light was noted when simulating the equipment exceeding 15 PPM. There was no alarm. MARPOL I/14.7

Detectors, manual alarm stations, control panels, cabinets, alarms, and other notifying devices must be of approved types. Vessel's detectors, manual alarm stations, control panels, cabinets, alarms, and other notifying devices have not been approved by MSC. In addition, current system does not match the vessel's alarm checklist IAW 46 CFR 95.05-1 which sends you to 46 CFR 76.27-5(a).

On CO2 systems in which the CO2 cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Vessel does not have a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate next to its CO2 system.

Every marine diesel engine installed on board a ship shall be provided with a Technical File. The Technical File shall be prepared by the engine manufacturer and approved by the Administration, and required to accompany an engine throughout its life on board ships. IAW MARPOL Annex VI Reg 13.8 and NOx Technical Code 2008.

Each ship subject to reg 6.1 that has rechargeable systems that contain ozone-depleting substances shall maintain an ozone-depleting substances record book.

There are multiple wire penetrations throughout vessel in bulkheads intended to be watertight. Rectify penetrations identified throughout vessel to make watertight IAW the vessel's stability booklet.

Except as provided in paragraph (d) of this section, after loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master or person in charge shall determine that the vessel complies with all applicable stability requirements in the vessel's trim and stability book, stability letter, Certificate of Inspection, and Load Line Certificate, as the case may be. Prior to next intended voyage, illustrate stability calculations to an attending CG Marine Inspector as per the vessel's stability book.

Fire, man overboard, and abandon ship drills to be conducted prior to next intended voyage with the attendance of a CG Marine Inspector.

Each alleyway, stairway, and exit giving access to a muster and embarkation station must be adequately illuminated by lighting that is capable of having its power supplied by the vessel's emergency source of electrical power. Provide emergency lighting in way of embarkation location.

Replace fire hose located in engine room.

Rescue boat navigation light detached. Repair as appropriate IAW 46 CFR 160.156.

All enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. Vents in accommodation spaces are missing vent covers and ability to close.

Prove the proper operation of the pilothouse steering control system. Jarring and erratic rudder movement was noted during the operational test from the pilothouse full followup control.

Prove the accuracy of the rudder angel indicating system. The pilot house indicator had a deviation of more than 10 degrees from the rudder position.

Prove the proper operation of the bilge and ballast system. The fixed system was unable to obtain suction from all areas served. IAW 46 CFR 56.50-50, 55, 57

Prove the proper operation of the sea chest's skin valve. It was seized in an open position.

Replace all nonmetallic piping in way of the fuel oil supply and transfer system in excess of 30" and not installed for flexibility.

Replace the failed pressure relief for the center start air tank. The relief failed to reset after testing.

Remove the temporary hose installed on the A-frame case drain into the HPU located in the aft machinery space and provide permanent piping and proper penetrations.

Replace the deteriorated bilge strainer located in the centerline forward engine room.

Domestic oceans/coastwise vessels of more than 400 GRT must have an approved OWS. The installed OWS continually failed in an alarm condition. Prove proper operation of the OWS and provide proof of the 5-year OCM calibration. 33 CFR 155.360

Repair and prove the proper operation of the inoperative STBD day-tank F/O shut-off valve. 46 CFR 56.50-75

The 1FEB2016 to Present Oil Record Book was missing weekly soundings of the sludge tank (last 9JUN2017), disposal of oily waste was entered under the part II (J) designation for cargo and no reception facility receipts were available. 33 CFR 151

The electronic propulsion control system is feed from a single 24vdc power source. The secondary means is a AC to DC power supply/charge however the main feeder panel is feed solely from the battery bank. Failure of the secondary source (batteries) resulted in a lost of propulsion while the primary system was available. Prove independent operation of the primary and secondary power system. 46 CFR 58.05-10 46 CFR 62.30(c)

Repair the cavitating No.2 steer gear hydraulic pump. 46 CFR 58.25-10

Repair the faulted electric engine order telegraph system. 46 CFR 113.35-7

Secure and properly support the e-lighting batteries located above the engine room egress ladder. 46 CFR 111.15-5

Conspicuously place a “discharge of oil prohibited” placard at the bilge water overboard discharge outlets and pump controls. 33 CFR 155.450

Provide an updated circuit designation to include equipment served and rating of each over current device on the main switch board. 46 CFR 111.40-11

Each emergency communication system (sound power phone) in the machinery space must be in a type 2 NEMA or IP 22 enclosure. The head set in the main machinery space was stored openly and headset was missing out of the auxiliary machinery space (steering gear room). A booth or other equipment must be provided in a noisy location to permit reliable voice communication. No sound dampening device was installed in way of the steering gear hydraulic control unit located in the main machinery space. Each sound power phone connection box must be type 4 or 4X NEMA or IP 56. The current installation consists of a single cable with terminals. 46 CFR 113.30-25

Each diesel engine must be provided with overspeed and low lube oil safety auto-shutdowns. The provided technician exclaimed the Cummins (2008) MDEs were not set-up. 46 CFR 58.05-10

An audible and visual alarm indicating a loss of phase, motor overload, and low oil for the steering system is required in the machinery space. Provide alarms. 46 CFR 58.25-25

Access to the steering compartment is through a duct tunnel and not readily accessible and the steering gear hydraulic units are located in the main machinery space. Steering gear compartments must be readily accessible and the steering gear hydraulic units are required to be installed outside the main machinery space. 46 CFR 58.25-40

Provide a means to process bilge water from the forward bow thruster compartment and the after steering gear compartment through the OWS or a means to discharge oily water from these compartments to a reception facility. The OWS only takes suction from the main machinery space and the common bilge system only provides overboard discharge. 33 CFR 155.360

Update automation test procedures to capture the additional required alarm and safeties for approval. 46 CFR 15.825

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work except as modified by 46CFR91.45-1. The general scope of the plans shall be noted in 91.55-5. The vessel does not have a up to date General Arrangement Plan, Subdivision and stability Plan, Fire Control Plan (to include HALON activation and shutdowns procedures), Marine Engineering Plan, Electrical Engineering Plan, Lifesaving Equipment (Embarkation plan) and Crew Accommodations or Navigation Bridge Visibility. 46 CFR 91.55-5a

Each vessel which in the ordinary course of its trade makes voyages of more than 3 days duration between ports and which carries a crew of 12 or more, must be provided with a hospital space. 46 CFR 92.20-35 a

Each watertight door which is in a required subdivision bulkhead, an indicator light must be installed in the pilothouse and at each other vessel operating station from which the door is not visible. WTDs do not have any indicator to show if they are open of closed. 46 CFR 170.270 e

Each Class 1 door must have a quick action closing device operative from both sides of the door. All WTD require 6 dogs to be secured. 46 CFR170.270 a

Electric installations on vessels must (1)ensure maintenance of services necessary for safety under normal and emergency conditions and (2) the protection of passengers, crew and other persons from electrical hazards. Inspectors noted countless dead end cables, improper plastic brackets, missing supports, improper installations of junction box and cable penetrations. 46 CFR 111.01-1 a

The items listed in 46CFR160.041-4(b) shall be in the first-aid kit. Random medicine kits on board were found to have expired meds. Eye was stations were found to have expired eyewash. 46 CFR 160.041-4 b

Each vessel must have an amplifier-type announcing system that will supplement the general emergency alarm. If a decentralized-type system is used, its overall performance must not be affected by the failure of a single call station. Vessels laud hailer has not operated in years. 46 CFR 113.50-5 a

Each launching station, or each two adjacent launching stations, must have an embarkation ladder as follows: (1) Each embarkation ladder must be approved under approval series 160.117 or be a rope ladder approved under approval series 160.017.(2) Each embarkation ladder must extend in a single length from the deck to the waterline with the vessel in its lightest seagoing condition under unfavorable conditions of trim and with the vessel listed not less than 15 degrees either way. 3) Provided that there is at least one embarkation ladder on each side of the vessel, the OCMI may permit additional embarkation ladders to be other approved devices that provide safe and rapid access to survival craft in the water. (4) The OCMI may accept other safe and effective means of embarkation for use with a life raft required under § 199.261(e). Vessel does not have sufficient embarkation ladders. 46 CFR 199.110 f

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master or person in charge shall determine that the vessel complies with all applicable stability requirements in the vessel's trim and stability book, stability letter, Certificate of Inspection, and Load Line Certificate, as the case may be. The vessel may not depart until it is in compliance with these requirements. Crew has not been ensuring the vessel's stability prior to sailing, as per the stability book. 46 CFR 97.15-7

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order. The pull tab on the fire blanket, located in the galley was found unattached. 46 CFR 199.190 a

Each vessel must carry not less than 12 rocket parachute flares, stowed on or near the bridge. Vessel did not have the correct Distress signals on the bridge. 46 CFR 199.60 c

Lifesaving appliances and arrangements are subject to Initial inspection test in 199.45 a, whenever a wire fall for a launching appliance is replaced or turned end to end. The rescue boat's falls were renewed and must be tested (110%) prior to getting underway. 46 CFR 199.60 C 1

Both generators must be fitted with nameplates made of corrosion resistant material that contain all of the information required in ABS Steel Vessel Rules section 4-8-3. This eficiency must be rectified in 45 days.

Complete drills, security, and other remaining inspection items with Coast Guard inspector. 46 CFR 91.27-1.
