Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. - 46 CFR 143.205(a) Vessel reported a loss of propulsion on the port engine two times due to unknown causes. Vessel must provide TPO report attesting to the satisfactory operation of the propulsion system.
Action required: 60 - Rectify deficiencies prior to movement
