Whenever an accident occurs to a ship or a defect is discovered, either of which affects the safety of the ship, the master shall report at the earliest opportunity to the administration or recognized organization (RO) responsible for issuing the relevant certificate; vessel lost propulsion while inbound on the cape fear river. Vessel must provide class survey report attesting to problem which caused the main engine to fail and an attestation that main engine is operable and reliable.

Resolution: Vessel replaced the #3 fuel oil pressure switch and cleanded the fuel oil drain valve. Class LR surveyor attended and verified repair.
