The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of relevant rules, regulations, codes and guidelines. Subject to Regulation 3 of this Anne, an emission control area designated for Tier III NOx control under paragraph 6 of this regulation. The operation of a marine diesel engine that is installed on a ship is prohibited except when the emission of oxides from the engine is within limits when that ship is constructed on or after 1 Jan 2016 and is operating in the North American Emission Control Area. EGR was not used on inbound transit to Tacoma while in the North American Emission Control Area. EGR is required to operate to meet TIER III NOx emission requirements.

In case of shut off of the gas fuel supply, the engines shall be capable of continuous operation by fuel oil only without interruption. An automatic system shall be fitted to change over from gas fuel operation to oil fuel operation and vice versa with minimum fluctuation of the engine power. Acceptable reliability shall be demonstrated through testing. There are various issues that currently prevent all three of the auxiliary engine from running on gas mode, so the changeover from gas fuel to diesel fuel could not be observed/tested (with associated master gas valve closure, etc.)

Direct access shall not be permitted from a non-hazardous area to a hazardous area. Where such openings are necessary for operational reasons, an airlock which complies w/ 5.12 shall be provided. An audible and visual alarm system to give a warning on both sides of the airlock shall be provided... Vessel's operations manual indicates LNG hold space as a non-hazardous area and the Tank Connection spaces as hazardous. PSCO discovered air lock between these two spaces without an audible and visual alarm when more than one door is moved from a close position.
