The safety of the towing vessel is the responsibility of the master and includes: Adherence to the provisions of the COI; Compliance with the applicable provisions of this subchapter; supervision of all persons onboard in carrying out their assigned duties.
Vessel master was unaware his responsibilities. Vessel master also was improperly loggin weekly checks of lifesaving and firefighting equipment.

Smoke detection in berthing spaces. Each towing vessel must be equpped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. This may be accomplished by an installed detection system or by using individual battery-operated detectors meeting UL 217.
Vessel smoke detector was inop and not UL 217 approved.

Fire Detection system: The system draws power from two sources. Switchover from the primary source to the secondary source may either manual or automatic.
Vessel fire detection panel was inop as the fuse was removed and batteries were unplugged.
Master gun decked the log showing that the item had been checked that morning. Vessel rep stated the fuse was removed due to previous hot work a few week prior. Replaced fuse and battery connections.
