Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500l and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. PSCO observed emergency generator remote valve inoperable.

The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the Company. The technical and/or operational nature of the deficiencies noted in deficiencies 01 & 02 above provide objective evidence of a serious failure of the implementation of the ISM code. An internal audit report must be submitted to the Coast Guard and deemed satisfactory prior to clearance of this deficiency.

Inspection of the life-saving appliances, including lifeboat equipment, shall be carried out monthly using the checklist required by regulation 36.1 to ensure that they are complete and in good order. A report of the inspection shall be entered in the log-book. Vessel had no record of inspection of rescue boat life-saving appliances.

PSCO identified that emergency exit in Bosn Stores not marked for emergency exit.
