13104 - On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. The visual indicator for the high level alarm was inoperable. 46 CFR 182.530(a)

11117 - Each ring life buoy must be marked with Type II retroreflective material. The retrorefelective tape on the ring life buoy was deteriorated. 46 CFR 185.604(i)

09110 - Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. There were dead end wires in the cabin behind the console. 46 CFR 183.200(b)

All vessels shall be provided with bilge pumps in accordance with Table 182.25-10(a). A submersible electric bilge pump may be used as a power bilge pump on certain vessels provided that: a positive shutoff valve is installed at the hull penetration. The subject vessel is an "old T" vessel and has individual electric submersible bilge pumps in lieu of the fixed power bilge pump required by Table 182.25-10(a). If electric submersible bilge pumps are used, the applicable requirements in "new T" must be met. The pump discharge line for the aft submersible electric bilge pump is missing a positive shutoff valve. Additionally, the flexible hose discharge line was found disconnected from the hull and any water to be discharged would have discharged back into the vessel. Fit a positive shutoff valve on the discharge line as near to the hull penetration as practicable, and connect the discharge piping so any water from the bilge line discharges overboard. 705c - due in 3 months' time (2/16/24)

The owner, charterer, master, or managing operator shall instruct each crew member upon first being employed and prior to getting underway for the first time on a particular vessel, and at least once every 3 months, as to the duties that the crew member is expected to perform in an emergency, including, but not limited to, the emergency instructions listed on the mergency instruction placard required by 185.510 of this part. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Fire fighting drilsl and training shall be logged or otherwise documented for review by the Coast Guard upon request. The last recorded fire and man overboard drills/training onboard the subject vessel at the time of the annual COI inspection were from 2021. *Fire and man overboard drills were carried out during the inspection and the crew requested to log these drills, and ensure training/drills are carried out at <3 month intervals moving forward.

Child life jacket's stowage location was not labeled.

The owner or managing operator shall make a vessel available for drydock examinations and internal structural examinations as required in 46 CFR 176.600. Due to your vessels current operational status, you failed to complete your required Drydock and Internal Structural Exam. Complete Credit Drydock and Internal Structural Exam prior to the carriage of passengers.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from machinery failure and personnel injury. Rudder packing leaking excessively and not maintaining suitable watertight integrity for the protection of the vessel. 46 CFR 182.100.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts, U.S. Coast Pilot [name withheld] List. Vessel missing U.S. Coast Pilot, Charts have not been updated since 2017, and Coast Guard Light List has not been updated since 2018. 46 CFR 184.420(a)

Each child lifejacket must be stowed in a location that is appropriately marked and separated from adult lifejackets so the child size lifejackets are not mistaken for adult lifejackets. Child size lifejackets are stowed forward and immediately next to the adult lifejackets, with no markings identifying the separation between the stacks of jackets. 46 CFR 180.78(a)(5)

Fuel shut off valves shall be installed so as to be operable from outside the tank compartment. Shut valve located on the engine was seized and not operable. Provide this office with proof of operable fuel shut off valve.

All vessels shall be provided with an approved adult type life preserver for each person carried. Vessel had 3 lifejackets that were not in good and serviceable condition. Provide this office with proof of 3 additional lifejackets.

A vessel must carry an approved first aid kit. Vessel's first aid kit contained multiple expired items including medicines and eye wash. Provide this office with proof of replacement items for the expired medicines and eye wash.
