The master shall conduct sufficient abandon ship drills, recovery of persons who have fallen overboard, and fire drills, to make sure that each crew member is familiar with his or her duties. During inspection there was not a licensed master to conduct drills. Vessel must have a licensed master and conduct drills to the satisfaction of the attending MI prior to departure.

There must be an individual holding an appropriate license as or a valid MMC with endorsement as master in command of each self-propelled inspected vessel. Crew on board could not produce a valid credential. Vessel must provide a valid credentialed master to the satisfaction of the attending MI prior to departure.

The owner or master shall instruct each crewmember, upon first being employed for the first time on a particular vessel and at least once every three months as to the duties the crewmember is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard. Crew training shall be logged or otherwise documented. The training entry shall include the date of the training and the general description of the training topics. The vessels owner or master could not provide logs or evidence of training. Additionally, the vessels crewmember was unfamiliar with his duties for these emergencies and did not receive training onboard the vessel. Vessel shall conduct training prior to getting underway to the satisfaction of the Coast Guard. 46 CFR 185.420(a) & (c)

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties. Fire Fighting drills and training shall be logged or otherwise documented. The drill entry shall include the date of the drill and training and the general description of the drill scenario and training topics. The vessels master could not provide logs or evidence of firefighting drills or training. Additionally, the vessels crewmember was unfamiliar with his duties for this emergency and did not receive training onboard the vessel. Vessel shall conduct drills and training prior to getting underway to the satisfaction of the Coast Guard. 46 CFR 185.524(a) & (d)

The master shall conduct sufficient drills and give sufficient instruction to make sure that all crewmember are familiar with their duties during emergencies that necessitate abandon ship or the recovery of persons who have fallen overboard. Abandon ship drills and training shall be logged or otherwise documented. The drill entry shall include the date of the drill and training and the general description of the drill scenario and training topics. The vessels master could not provide logs or evidence of man overboard or abandon ship drills or training. 46 CFR 185.520(a) & (f)

Emergency instructions are not designed to address the particular equipment, arrangement, and operation of the vessel, and do not address flooding as required and must be updated. 46 CFR 185.510(b)

Each lifejacket must be marked with Type I reflective material approved under approval series 164.018. 01 Lifejacket was discovered to have worn retro-reflective material. Vessel shall repair material or replace lifejacket. 46 CFR 185.604(h)

Ring life buoys need retroreflective material replaced to meet approval series 46 CFR 160.150 as required. 46 CFR 180.70(b)(1)

Vessel must have loading marks and/or draft marks painted in a contrasting color to the sideshell paint. 46 CFR 185.602(d)

A unauthorized repair was discovered on the pressure vessel located in the starboard side engine room. A bunghole on the top of the pressure vessel was slugged and welded closed. Pressure vessel will need to be replaced, or hydrostatically tested.
