Every ship of 100GT and above, and every ship which is certified to carry 15 or more persons, and fixed or floating platforms shall carry a garbage management plan which the crew shall follow. This plan shall provide written procedures for minimizing, collecting, storing, processing and disposing of garbage, inlcuding the use of the equipment onboard.
PSCO observed all locations for storing garbage full, including the garbage rm, freezer, engineering workshop. Vessel received stores creating additonal garbage and didn't intend on discharging until after 5 day voyage to Panama.

The time time if inspection, PSCO was briefed by crew that stevedores from Alabama State Docks punctured the port side of #2 Cargo Hold into #2 Port ballast tank with a forklift resulting in the ingress of water into the cargo hold.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Annual servicing of the fixed CO2 system was conducted on Aug. 18, 2018 by a 3rd party servicing technician. The technician report indicated 03 items "were found non-conforming and require service" but servicing was not conducted. Those items, including 05-45kg CO2 cylinders of main system for E/R and cargo holds needed recharging; the single 45 kg CO2 cylinder for the EDG room needs replacing or reconditioning; and the single 45 kf CO2 cylinder for the paint locker was condemned for excess rust on bottom. Those items were not completed during servicing and PSCO found the cylinders for the EDG room and paint locker to have "condemned" stickers prominently placed on the cylinder. 74 SOLAS (14) II-2/14.2.1.2

The vessel experienced multiple losses of propulsion during transit inbound on the Cape Fear River due to exhaust gas from cylinder #6 leaking into the HT cooling water, causing the main engine to shutdown. 74 SOLAS (14) I/11(c)
