07106 - Fire Detection and alarm system Internal conbustion engines of 2,250 kW and above or having cylinders of more than 300 mm bore shall be provided with crankcase oil mist detectors or engine bearing temperature monitors or equivalent devices. PSCO observed alarm for oil mist detector system. 74 SOLAS (2004) II-1/47.2

11102 - Lifeboat inventory A food ration consisting of not less than 10,000 KJ for each person the lifeboat is permitted to accomodate. PSCO observed expired food rations on free-fall lifeboat. New rations scheduled for delivery in Houston. 74 SOLAS (2020) III/34 LSA Code 4.4.8.12

07106 - Fire Detection and Alarm System The arrangements provided shall be such to ensure that the safety of the ship in all sailing conditions is equivalent to that of a ship having the machinery spaces manned. PSCO observed alarm for oil mist detection system with no additional measures in place regarding manning. 74 SOLAS(2004) II-1/46.1

Every ship of 100GT and above, and every ship which is certified to carry 15 or more persons, and fixed or floating platforms shall carry a garbage management plan which the crew shall follow. This plan shall provide written procedures for minimizing, collecting, storing, processing and disposing of garbage, inlcuding the use of the equipment onboard. PSCO observed all locations for storing garbage full, including the garbage rm, freezer, engineering workshop. Vessel received stores creating additonal garbage and didn't intend on discharging until after 5 day voyage to Panama. MARPOL Annex V / Reg 10.2

Spare sprinkler heads shall be provided for each section of sprinklers to the satifaction of the Administration. PSCO observed vessel missing spare sprinkler heads for the purifier flat sprinkler system. 74SOLAS(2001)II-2/12.10

Fire-extinguishing appliances shall be kept in good working order and be available for immediate use at all times. PSCO observed all CO2 bottles for the engine room pinned, preventing immediate use. 74 solas (01) II-2/21 17ac

Every lifebuoy shall, if it is intended to operate the quick-release arrangement provided for the self-activated smoke signals and self-ignighting lights have a mass of not less than 4kg. PSCO observed STBD bridge wing lifebuoy weighing 2.5kg and both port and stbd quick release arrangements corroded/painted over preventing operation. 74 solas (20) III/34 LSA 2.1.1.7 17ac/10c

Each completed page or group of electronic entries shall be signed by the master of the ship. PSCO observed 02 pages of the Oil Record Book Part 1 not signed by master. marpol I/17.4 10c

For a period of 3 hours, emergency lighting at every muster and embarkation station and over the sides as required by regs III/11.4 and III/16.7. PSCO observed multiple inoperable bulbs along port side emergency lighting.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. The time time if inspection, PSCO was briefed by crew that stevedores from Alabama State Docks punctured the port side of #2 Cargo Hold into #2 Port ballast tank with a forklift resulting in the ingress of water into the cargo hold.

..The detection system shall initiate audible and visual alrms distinct in both respects from the alarms of any other system not indicating fire, in sufficient places to ensure that the alarms are heard and observed on the navigation bridge and by a responsible engineering officer. PSCO noted the fire detection system in the engine room was not clearly indicating a fire alarm in the engine room via the correctly indicated beacon.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Annual servicing of the fixed CO2 system was conducted on Aug. 18, 2018 by a 3rd party servicing technician. The technician report indicated 03 items "were found non-conforming and require service" but servicing was not conducted. Those items, including 05-45kg CO2 cylinders of main system for E/R and cargo holds needed recharging; the single 45 kg CO2 cylinder for the EDG room needs replacing or reconditioning; and the single 45 kf CO2 cylinder for the paint locker was condemned for excess rust on bottom. Those items were not completed during servicing and PSCO found the cylinders for the EDG room and paint locker to have "condemned" stickers prominently placed on the cylinder. 74 SOLAS (14) II-2/14.2.1.2

The company is responsible for ensuring that adequate resources and shore based support are provided. The ship's captain sent a "Request for Repair/Service" to company to rectify items found by servicing technician during annual servicing of the fixed CO2 system on Aug. 18, 2018. Company responded by instructing the vessel to close the request and the items would be addressed during the "next annual verification." 74 SOLAS (14) IX/5 and ISM Code 3.3

The master shall report immediately to port state control authorities whenever an accident occurs to a ship or defect is discovered, which affects the safety of the ship. The vessel experienced multiple losses of propulsion during transit inbound on the Cape Fear River due to exhaust gas from cylinder #6 leaking into the HT cooling water, causing the main engine to shutdown. 74 SOLAS (14) I/11(c)
