07199 - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will permit fit to proceed to sea without danger to the ship or persons on board. PSCO observed active exhast leaks on #1 and #2 aux gen. Additionaly multiple oil fuel leaks where found on each aux generatiors and purifiers. 07105 - Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight and self closing. PSCO observed multiple doors leading from engine room to ECR do not close properly. 74 SOLAS (01) II-2 / 47.1 30AC

15108 - The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. PSCO's observed deck water spray piping burst at 3 different locations. Additionally multiple explosion proof boxes were found wasted or damaged rendering them ineffective. The boxes were found throughout the deck area. Due to the objective evidence in the below deficiencies, the vessel is not in compliance with relevant conventions, calling into question the adequacy and implementation of the vessel's SMS under the ism code. An external audit conducted by the flag or RO is required within 30 days to determine whether the ship is operating in accordance with the ism code. Provide a copy of the complete audit report to USCG prior to departure from port. 74 SOLAS (20) IX/3.1 ISM CODE 10.1 30bc

07101 - On ships carrying flammable or toxic products or both, a water spray system for cooling fire prevention and crew protection should be installed. PSOC observed main water spray pipe burst in 03 areas; portside compressor room, port side tanks, and starboard side motor room near manifold. The bursted pipe renders the system ineffective. 74 SOLAS(20)VII / 13.1 IGC(93) 11.3.1 30bc

06107 - Where electrical equipment is installed in gas-dangerous spaces or zone, it should be to the satisfaction of the administration and approved by the relevant authorities recognized by the administration for operation in flammable atmosphere concerned. PSCO observed multiple explosion proof boxes on deck where missing bolts and/or gaskets were wasted, ultimately the integrity of those boxes were compromised. Additionally multiple cable glands were found damaged or missing. 74 SOLAS (20)VII / 13.1 IGC(93) 10.1.5 30bc

11104 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order ready for immediate use. Rescue boat engine would not start after 30 minutes of repeated starting attempts and maintenance. 74 SOLAS(20)III/20.2 17ac

11132 - The following tests and inspections shall be carried out weekly and a report of the inspection shall be entered in the logbook. Engines on rescue boats shall be run for a total period of not less than 3 minutes. The logbook tracking rescue boat preventative maintenance had no logged weekly inspections since the 4th week of June 2023. 74 SOLAS (20) III/20.2.6.2 17ac

01214 - Subject to the provisions of regulation i/10, any certificate required by the convention must be kept available in its original form on board the ship on which the holder is serving. PSCO discovered that none of the licensed mariners on the vessel had original flag state endorsements on board. STCW REG I/2-11 17bc

07106 - The function of the detection system shall be periodically tested to the satisfaction of the administration by means of equipment producing hot air at the appropriate temperature. PSCO observed that the vessel had no authorized means of testing their heat detectors onboard. 74 SOLAS(01)II-2/13.1.13 17abc

08103 - Any required fixed fire detection and fire alarm system with manually operated call points shall be capable of immediate operation at all times. PSCO observed that the smoke detectors were not operating in the purifier room. Additionally, the call point on deck b’s main stairwell was broken. 74SOLAS(01)II-2/13.1.1 17ac

11117 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed a cracked lifebuoy on the starboard bridge. 74 SOLAS (20) III/ 20.2 17AC

06105 - Gas detection equipment should be so designed that it may readily be tested. Testing and calibration should be carried out at regular intervals. Suitable equipment and span gas for this purpose should be carried on board. PSCO observed crew using a span gas bottle for the fixed gas detection system without a flow regulator meeting the approved testing procedures. 74 SOLAS (20) VII / 13.1 IGC (93) 13.6.6 17AC

06107 - The condition of the ship and its equipment should be maintained to conform with the provisions of the code to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons on board or without presenting unreasonable threat of harm to the marine environment. PSCO observed pressurized cargo release while testing ESD on starboard manifold. Crew admitted to loosening cargo manifold flange in preparation for cargo operations. 74 SOLAS (20) VII / 13.1 IGC (93) 1.5.3.1 10C

(1870) Suitable glands should be provided where the electric motor shaft passes through the gastight bulkhead for driving cargo compressors. PSCO observed lubrication oil under all (4) electric motor shafts in the motor room indicating leaking at the gland. 74 SOLAS 01 Con. II-2/55.6, IGC Code 10.2.4.2

(1870) Mechanical ventilation inlets and outlets should be arranged to ensure a safe working environment. 5 of 6 exhaust ducts (low level) were found with inoperable flaps during an operational test. 74 SOLAS 01 Con. II-2/55.6, IGC Code 12.1.2

(1899) Every steam pipe and every fitting connected thereto through which steam may pass shall be constructed and installed to withstand the maximum working stresses to which it may be subjected. PSCO discovered steam leak at pipe connection in gas dangerous zone in vicinity of centerline catwalk forward of the motor room, on deck. 74 SOLAS 01 Con. II-1/33

(1870) Where electrical equipment is installed in gas-dangerous spaces or zones, it should be to the satisfaction of the administration. PSCO found grounding wires disconnected on the following equipment located in a gas dangerous zone: starboard booster pump, deepwell pump tank number one. 74 SOLAS 01 Con. (0699) Alleyways giving access to embarkation stations shall be lighted. Three lights were found inoperable in the centerline catwalk providing safe access to tanker bow and six person liferaft located forward. 74 SOLAS 01 Con. III/11.5

(0730) Ships in which carriage of flammable products is intended should be fitted with fixed dry powder type extinguisher systems for the puropse of fighting fire on deck in cargo area. PSCO found valve handle inoperable due to corrosion on powder gun number two port side, located in the vicinity of tank number two. 74 SOLAS 01 Con. II-2/55.6 ICG Code 11.4.1

ACCESS TO THE LIFEBOAT PROVIDED BY HATCHES WHICH CAN BE CLOSED TO MAKE THE LIFEBOAT WATERTIGHT. THE GASKET AROUND THE HATCH IS DAMAGED, PREVENTING THE LIFEBOAT FROM BEING WATERTIGHT. 40AC
