PSCO observed active exhast leaks on #1 and #2 aux gen. Additionaly multiple oil fuel leaks where found on each aux generatiors and purifiers. Resolution: RINA surveyor observed satisfacory operation. When PSCO went to test the doors, PSCO observed doors were not closing properly. Issue brought up to Master, vessel will be going to dry dock and fixing issue at that time.

15108 - The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. PSCO's observed deck water spray piping burst at 3 different locations. Additionally multiple explosion proof boxes were found wasted or damaged rendering them ineffective. The boxes were found throughout the deck area. Due to the objective evidence in the below deficiencies, the vessel is not in compliance with relevant conventions, calling into question the adequacy and implementation of the vessel's SMS under the ism code. An external audit conducted by the flag or RO is required within 30 days to determine whether the ship is operating in accordance with the ism code.
