Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Battery charger has blade type fuses installed. Replace with cartridge type fuses or remove battery charger to satisfaction of attending marine inspector. 46 CFR 183.380

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: The pump is not permanently mounted.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Analgesic medication expired. Provide 48 doses of analgesic medication in first aid kit.

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Waterlight stowed too far from ring life buoy to attach lanyard when stowed. Relocate to properly attach waterlight lanyard to ring life buoy.

Each floating electric waterlight shall meet the requirements of ANSI/UL 1196. A stroboscopic-flashing or flashing-incandescent waterlight shall emit flashes of light at a frequency of not less than 50 nor more than 70 flashes per minute for not less than 15 hours. Floating water light not flashing as frequent as required. Replace floating water light to satisfaction of attending marine inspector.

A vessel must be provided with a main steering gear that is: (1) Of adequate strength and capable of steering the vessel at all service speeds. Vessel lost the ability to steer while underway. Vessel to prove proper operation of the steering assembly to the satisfaction of an attending marine inspector.

A submersible electric bilge pump may be used as a power bilge pump. The pump is listed by an independent laboratory as meeting the requirements in UL 1113. Bilge pumps were not UL rated. Replace with UL rated bilge pumps. 46 CFR 182.520(e)(1)

Spaces containing machinery shall be fitted with at least two ducts to furnish natural or mechanical supply and exhaust ventilation. One duct shall extend to a point near the bottom of the compartment, so installed that the ordinary collection of water in the bilge will not trap the duct. Where forced ventilation is installed, the duct extending near the bottom shall be the exhaust. The total inlet area and the total outlet area of ventilation ducts shall each be not less than one square inch for each foot of beam of the vessel. These minimum areas shall be increased as necessary when such ducts are considered as part of the air supply to the engines. Crew members could not identify natural ventilation locations for engine room. Locations were not readily apparent and did not have capability of securing ventilation. Submit proposal to marine inspector with plan for new supply/exhaust installations to be installed next dry dock. 46 CFR 182.20-45

Where a flexible vent pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. Flexible hose has a kink and is not suitable for intended use. 46 CFR 182.450(f)

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Fresh water PVC valve actively leaking into engine room. 46 CFR 176.800(a)

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Vessels waterlight lanyard wasted. Replace lanyard. Vessel restricted to daylight operations only. 46 CFR 180.71

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. The emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in § 185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessel did not have emergency instructions posted. Post emergency instruction. 46 CFR 185.510

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel did not have life jacket placard posted. Post life jacket placard. 46 CFR 185.516 46 CFR 185.516

Each vessel must be marked as required by part 67, subpart I, of this chapter. Vessel name not marked on port and starboard bow. Mark port and starboard bow with vessel's name. 46 CFR 185.602

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A space with a through-hull fitting below the deepest load waterline, such as a lazarette; a machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and a space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. The vessel does not have high water alarms in port or starboard demi hulls. Install high water alarms. Vessel does not have visual indication of high water alarm in aft engine space. Provide visual indication for high water alarm. 46 182.530(a)

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Vessel does not have MIS report from consortium. Vessel DER currently acting as captain of vessel. Provide MIS report. Provide DER with proper training who does not hold safety sensitive position on vessel. 46 CFR 185.212

Each vessel must - Have permanent draft marks at each end of the vessel; or have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum allowable trim and amidships to indicate the maximum allowable draft. A loading mark required by paragraph (c)(2) of this section must be a horizontal line of at least 205 millimeters (8 inches) in length and 25 millimeters (1 inch) in height, with its upper edge passing through the point of maximum draft. The loading mark must be painted in a contrasting color to the sideshell paint. Vessel does not have Load marks or draft marks. Provide proper vessel marking prior to next credit dry dock. 46 CFR 185.602(b)(c)

The space containing propulsion machinery must be equipped with a fixed gas fire extinguishing system in compliance with 181.410 of this subpart. Fixed gas fire extinguishing system did not shut down engines during operational test. Operator must repair prior to the carriage of passengers.

PROVIDE UPDATED CONTENTS IN FIRST AID KIT.
