Cables entering boxes or fittings shall be protected from abrasion, and shall conform with the following: Openings through which conductors enter shall be adequately closed. Fire pump junction box did not have proper enclosures for cable entries. 46CFR 183.10-10(a) (Old subchapter T)

The inspection shall be such as to insure that the materials, workmanship and condition of the vessel and its machinery and equipment are in all respects satisfactory for the service intended. Repair called out damage to vessel deck stringers and internal structures. 46CFR 176.05-5 (Old subchapter T)

To provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel circuits must be appropriately labeled in distribution panels and circuit breaker panels. 46CFR 183.01.5 (Old subchapter T)

Rotating or uninsulated machinery parts cannot be inadvertently touched or approached nearer than a safe distance by any person. Rotating machinery on the main engines were not appropriately protected. 46CFR 183.10-10 (Old Subchapter T)

Except as provided elsewhere in this subpart, all electrical equipment including motors, generators, controllers, distribution panels, etc., shall be dripproof protected. Dripproof protected means that equipment and machinery is so constructed or protected that its successful operation is not interfered with when subjected to falling moisture or dirt, and rotating or uninsulated parts cannot be inadvertently touched or approached nearer than a safe distance by any person. Electrical equipment in the aft portion of machinery space has exposed contacts. 46CFR 183.10-20(f)(1) (Old subchapter T)

Repairs must be inspected by a marine inspector. The following were noted as work list items during the vessel's scheduled dry dock and must be verified complete to get DD/ISE credit. Work list items: Identified additional work list items as follows: 31. Hole/wastage in deck outboard of starboard fuel tank. Crop and renew (C/R). NDT. 32. Hole found on side shell between starboard fuel tank and potable water tank - C/R & NDT. 33. Diesel fuel was found under the port side fuel tank at each end. Check/ change potential leaking shutoff valve. 34. Weld above MSD is wasted. Repair. 35. An area of overhead longitudinals frames above the MSD were cropped and not renewed. Renew longitudinal frames that were cropped.

A dead-end cable was found near the hydraulic control unit in the Engine space. - 46 CFR 183.01-5(a) (OLD T)

At each inspection, the marine inspector shall examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather tight decks. Starboard Rudder post stiffener was found wasted. - 46 CFR 176.25-5 (a) (OLD T)

At all inspections, special attention shall be paid to ensure that no fire hazards exist. Twelve total hydraulic hoses in the engine room, (6 on the port side, 6 on the starboard side) are currently cracked, wasted, and leaking hydraulic fluid. - 46 CFR 176.25-10(b)(OLD T)

Irrespective of build date, vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with Part 181.450 that is installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel had independent smoke detector not interconnected in wheelhouse. - 46 CFR 181.405(c) 09MAY2024: Vessel does not have interconnected fire system at all.

The marine inspector shall examine all accessible parts of the vessel’s equipment to be assured that they are in a satisfactory condition. Installed anemometer was not reading wind conditions at the time of the inspection. - 46 CFR 176.10-5(c)(OLD T)

At each inspection, the marine inspector shall examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather tight decks. Internal framing located in the aft section on both the port and starboard side of the vessel has significant wastage. Inspectors also noted a soft patch area underneath the deck sink that was plugged with epoxy and aluminum foil. 705 - Vessel to make permanent repairs at next drydock - 46 CFR 176.25-5 (a)(OLD T)

The Certificate of Inspection issued to a vessel describes the minimum manning requirements. A review of the vessel’s logbook showed that on multiple dates the vessel was operating outside of its required minimum manning. - 46 CFR 176.103

A suitable rescue boat shall be required. The rescue boat engine was not fully operational.- OLD T - 46 CFR 180.10-35

Each life preserver carried on a vessel engaged in ocean, coastwise, or Great Lakes service after June 30, 1980, must have a personal flotation device light that is approved under subpart 161.012 of this chapter. The life jacket stored on the navigation bridge had an extinguished light. OLD T - 46 CFR 180.25-20

The intent of this part is to insure that each vessel is provided with safe, readily available and sufficient firefighting equipment of the type necessary to extinguish any fires which are likely to be encountered in normal operation. The starboard machinery space ventilation damper had an air gap when closed. - Old T - 46 CFR 181.01-5

Wiring joints and splices shall be mechanically secure and made in junction boxes. Improper connections were made to the PA system via wire nuts. Connection needs to be made within a junction box. - Old T - 46 CFR 183.05-50

On vessels where the amount of carbon dioxide gas required in a fixed system can be supplied by one portable extinguisher, such extinguishers may be used. The protected space (paint locker) had an inadequate seal, which would allow for the extinguishing agent to escape. - Old T - 46 CFR 181.20-5

At each initial and subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition, and fit for the service for which they are intended. Lighting fixtures – multiple lights in the engine room space were found extinguished. - Old T - 46 CFR 167.25-15

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall Examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather deck. A forward escape hatch from the galley to the weather deck had a cable penetration made through the trunk that was not made weather tight. - Old T - 46 CFR 176.25-5

At all inspections special attention shall be paid to insure that no fire hazards exist. A hydraulic leak was found on a hydraulic flow cylinder in the engine room space. - Old T - 46 CFR 176.25-10

To maintain a valid COI, you must complete your annual inspection within 3 months before or after your anniversary date. Vessel was required to have the annual inspection done by 02JUN2023. Inspection was completed on 21JUN2023.

Each load test and inspection by the certifying authority must be recorded in the unit's Crane Record Book. Dates in Crane Record Book were pass due. 46CFR107.258(c)

A fixed gas fire extinguishing system aboard a vessel must be engineered to meet the requirements of subpart D. Seal on CO2 paint locker was insufficient. 46CFR181.410(a)(1)

Vessels anemometer was missing; replace with a new anemometer.

Fuel lines must be accessible and protected fomr mechanical injury; fuel hoses on port main diesel engine were makign contact with coolant line and were not protected from the engine's flywheel.

Each rescue boat must be marked with Type II retroreflective tape approved in accordancew ith 46 CFR 164.0918 or other standard specified by the commandant; Retroreflective tape on vessel's workboat was wasted.

No emergency broadcast placard was posted next to radiotelephone installation. Post emergency broadcast placard in accordance with 46 CFR 184.506 and 47 CFR 80.868.

The remote fuel shutoff required by 46 CFR 182.20-40 does not fully close the fuel tank shutoff valves. Prove proper operation of the emergency fuel shutoff reach rod assembly.
