For tankers of 20,000 Tonnes deadweight and upwards, the protection of the cargo tanks shall be achieved by a fixed inert gas system. PSCO observed nitogren inert gas system inoperable while attempting to test high temp shutdown. Crew noted it was due to overheating. Prior to conducting cargo operations vessel must demonstrate that IG system is fully functioning.

OBSERVED TEMPORARY VENTILATION DUCTING IN PLACE BOTH IN THE COMPRESSED AIR ROOM COOLING THE AFT COMPRESSOR AND THE TRANSFORMER ROOM PROVIDING COOLING VENTILATION TO THE ELECTRICIANS STORE. GENERAL ARRANGEMENTS PLANS SHALL BE PERMANENTLY EXHIBITED FOR THE GUIDANCE OF THE SHIP'S OFFICERS, SHOWING CLEARLY FOR EACH DECK MEANS OF ACCESS TO DIFFERENT COMPARTMENTS. PSCO OBSERVED AN ACCESS DOOR LEADING FROM THE ELECTRICIANS WORKSHOP TO THE ENGINE CONTROL ROOM THAT WAS NOT INDICATED ON THE FIRE CONTROL PLAN. ALSO OBSERVED DOOR FRAME WITHOUT A DOOR LEADING FROM ELECTRICIANS WORKSHOP TO ELECTRICAL STOOR ROOM WHICH WAS IDENTIFIED AS A DOOR ON THE FIRE CONTROL PLAN. 0725-PSCO observed more than 30 water spray nozzles were not functioning, certain spots where nozzles were marked on the diagram were plugged, and a valve connection was in place of a nozzle. Numerous water spray nozzles were missing or providing an insufficient water blanket. Prior to the carriage of propylene oxide, the vessel is required to rectify this deficiency to the satisfaction of the certificate issuing authority and submit a report attesting to the satisfactory operation of the water spray system to the coast guard.

The oily water separator (OWS) would not operate properly. a requirement was issued prohibiting the vessel from operating the OWS until the certificate issuing authority thoroughly test and attests to the proper operation of the OWS.

BLACK OIL WAS FOUND INSIDE OVERBOARD DISCHARGE PIPE DOWNSTREAM OF THREE-WAY VALVE. THIS LEADS TO THE COLCLUSION THAT THERS IS A FAILURE IN THE THREE-WAY VALVE OPERATION OR THE SENSING EQUIPMENT. VSL MUST HAVE CLASS COMPLETE A REPORT ATTESTING TO THE PROPER OPERATION OF THE OWS THREE-WAY VALVE AND SENSING EQUIPMENT.
