THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO CONFORM WITH THE PROVISIONS OF THE PRESENT REGULATIONS TO ENSURE THAT THE SHIP IN ALL RESPECTS WILL REMAIN FIT TO PROCEED TO SEA WITHOUT DANGER TO THE SHIP OR PERSONS ON BOARD. VESSEL REPORTED COLLISION WITH TUG BOAT, THE JUDGE, UPON TRANSITING UNDER PILOTAGE FROM ANCHORAGE TO OIL DOCK 8. VESSEL SUSTAINED TWO 12 X 8 INCH DENTS ON STARBOARD BOW IN VINCITY OF FR#79. PER VESSEL'S PLAN, DNV CLASS SURVEYOR IS TO CONDUCT INTERNAL AND EXTERNAL INSPECTION OF THE DAMAGED AREA OF THE HULL AND BALLAST TANK. VESSEL SHALL SUBMIT SATISFACTORY CLASS REPORT TO US COAST GUARD PRIOR TO DEPARTING CORPUS CHRISTI COTP ZONE. 74 SOLAS (04) I/11(a)

For tankers of 20,000 Tonnes deadweight and upwards, the protection of the cargo tanks shall be achieved by a fixed inert gas system. PSCO observed nitogren inert gas system inoperable while attempting to test high temp shutdown. Crew noted it was due to overheating. Prior to conducting cargo operations vessel must demonstrate that IG system is fully functioning.

PSCO OBSERVED MULTIPLE WIRE RUN BULKHEAD PENETRATIONS THROUGHOUT MACHINERY SPACES FILLED WITH UNKNOWN FOAM SPRAY ALONG WITH CARDBOARD USED TO HOLD FOAM IN PLACE DURING INSTALLATION. OBSERVED TEMPORARY VENTILATION DUCTING IN PLACE BOTH IN THE COMPRESSED AIR ROOM COOLING THE AFT COMPRESSOR AND THE TRANSFORMER ROOM PROVIDING COOLING VENTILATION TO THE ELECTRICIANS STORE.
