[name withheld] stated "it was being used to create repair pieces for ventilation." PSCO observed 3 Oxygen and 1 Acetylene bottles stored in forecastle in vicinity of the hot work area. Reviewed the procedures for hot work permits issued for the hot work being conducted in forecastle. Additionally, the vessel's SMS procedures states that hot work is prohibited within 15 meters of dangerous goods. During multiple voyages the vessel had dangerous goods stowed in Bay 01 and Bay 03. Vessel is prohibited from conducting hot work in forecastle until company provides corrective action plan with concurrence with administration approval.

The Company should establish procedures to ensure that the ship is maintained I.A.W SMS. PSCO found that Emergency Diesel generator the oil filter had not been Replaced I.A.W. manufacturers specifications and that the preventive maintenance procedures do not address such maintenance. ISM Code 10.1

THE SEAL BETWEEN THE BOTTOM OF THE ENGINE AND CRANKCASE HOUSING IS LEAKING EXCESSIVE LUBE OIL.
