15104-safety management systems (ISM)- The company should clearly define and document the master's responsibility with regard to; implementing the safety and environmental-protection policy of the company PSCO observed a hot work area that was created by the crew in forecastle. [name withheld] stated "it was being used to create repair pieces for ventilation." PSCO observed 3 Oxygen and 1 Acetylene bottles stored in forecastle in vicinity of the hot work area. Reviewed the procedures for hot work permits issued for the hot work being conducted in forecastle. Additionally, the vessel's SMS procedures states that hot work is prohibited within 15 meters of dangerous goods. During multiple voyages the vessel had dangerous goods stowed in Bay 01 and Bay 03. Vessel is prohibited from conducting hot work in forecastle until company provides corrective action plan with concurrence with administration approval.
