Stairways and lift trunks which penetrate more than a single deck shall be surrounded by at least "A-O" class division and be protected by self-closing doors at all levels. PSCO's observed that the self-closing door to the escape trunk in the E/R did not fully close and latch properly. 74 SOLAS (97) II-2/46.1

PSCOs observed 01 smoke detector in the engine room immediately outside of the ECR which did not alarm when tested multiple times.
