Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel. Vessel bilge piping was only fitted with a check valve and did not have a closing device permanently affixed to prevent water ingress. Repair at next drydock

Each towing vessel must be maintained and operated so as to minimize fire hazards. Fuel filters on both generators and both mains have unshielded plastic bowls. 46 CFR 144.405 705c - Prior to operations.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Fire alarm strobe light housing in engine room is damaged and exposes wiring. 46 CFR 143.205(a) 705c - Prior to operations.

Each escape hatch and emergency exit used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR." Engine room escape hatch is not labeled on the aft deck. 46 CFR 144.160 705c - Prior to operations.

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Vessel deckhands have not been pre employment drug tested and are not enrolled in the company's random drug testing consortium. 46 CFR 16.210

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage, including:(1) Charts, including electronic charts acceptable to the Coast Guard, of appropriate scale to make safe navigation possible. Towing vessels operating on the Western Rivers must have maps of appropriate scale issued by the Army Corps of Engineers or a river authority; (2) “U.S. Coast Pilot” or similar publication;(3) Coast Guard light list; and (4) Towing vessels that operate the Western Rivers must have river stage(s) or Water Surface Elevations as appropriate to the trip or route, as published by the U.S. Army Corps of Engineers or a river authority, must be available to the person in charge of the navigation watch. (c) Extracts or copies from the publications listed in paragraph (b) of this section may be carried, so long as they are applicable to the route. Vessels charts, light list, local notice to mariners were out of date or not updated. 46 CFR 140.705

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Each towing vessel operating on oceans, coastwise, or Great Lakes routes must have a means to take blood pressure readings, splint broken bones, and apply large bandages for serious wounds. Vessels first aid kit had expired items, replace all expired items. 46 CFR 140.435 & 46 CFR 160.041-4.

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship and that each person handling garbage follows the plan. (b) Each garbage management plan under paragraph (a) of this section must be in writing and - (1) Provide for the discharge of garbage by means that meet Annex V of MARPOL, the Act, and §§ 151.51 through 151.77;(2) Describe procedures for minimizing, collecting, processing, storing, and discharging garbage; and (3) Designate the person who is in charge of carrying out the plan. Vessel has no waste/garbage management plan onboard. 33 CFR 151.57 & 151.51(d).

All lifesaving equipment must be tested and maintained in accordance with the minimum requirements of 46CFR199.190. Each inflatable lifesaving appliance must be serviced within 12 months of each subsequent servicing. The life raft on board the vessel required servicing in DEC2021.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110. Lifebuoys on board do not have the approved floating electric water light attached.

At least one lifebuoy must have a lifeline, secured around the body of the lifebuoy. If more than one lifebuoy is carried, at least one must not have a lifeline attached. Both lifebuoys on board are fitted with lifelines.

Each towing vessel operating on oceans, coastwise, limited coastwise, or beyond 3 nautical miles from shore upon the Great Lakes must carry a Category 1, 406 MHz satellite Emergency Position Indicating Radio Beacon (EPIRB) that meets the requirements of 47 CFR part 80. EPIRB battery has expired.

Each immersion suit or anti-exposure suit must be marked in such a way as to identify the person or vessel to which it belongs. Multiple immersion suits on board are not marked with the vessel or person.

Immersion suits or anti-exposure suits must have the following attachments and fittings: (i) Each immersion suit or anti-exposure suit must have a life jacket light approved under approval series 161.112 securely attached to the front shoulder area of the immersion suit or anti-exposure suit. Immersion suits on board were not fitted with lights.

All portable fire extinguishers, semi-portable fire-extinguishing systems...must be inspected or tested at least once every 12 months. Fire extinguishing equipment required service in NOV2021.

Verify that all observed unsafe practices and other hazardous situations are corrected. Means of escape from engine room has been sealed for weatherization and is unable to be opened from within the space.

When using DSC techniques, coast stations and ship stations must use maritime mobile service identities (MMSI) assigned by the Commission or its designees. Vessels DSC VHF radios do not have MMSI information programmed into devices.

Each U.S. towing vessel of 26 feet (about 8 meters) or more in length, must hold a ship-radio-station license for radio transmitters (including radar and EPIRBs), and each operator must hold a restricted operator's license or higher. No proof of restricted operators license (or higher) was on board the vessel for designated operator.

Unless otherwise provided by this subpart, each vessel required to have a Certificate of Inspection is subject to the annual vessel inspection fee listed in table 2.10-101 for its vessel category. User fees have not been paid since the original COI inspection.

The owner and managing operator must ensure that the towing vessel is in compliance with this subchapter and other applicable laws and regulations at all times. Vessel was not in compliance with multiple laws and regulations at time of inspection.

Provide valid FCC restricted operator's license or higher for all Masters and Mates.

Vessel needs to provide means to energized fire pump remotely from a safe place outside the engine room

Install a check/foot valve in each bilge suction that prevents unintended back flooding through bilge piping.

Permanently mount Life raft to allow float-free launching. The arrangement must ensure that the liferaft, when released and inflated, are not dragged under by the sinking vessel.

Label both fuel oil shutdowns

Until Vessel has provided proper operation of sewage system vessel is reduced to 12 hours of operation and must provide temporary means for holding sewage onboard.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the 100% of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.
