Vessels navigational charts and publications were not up to date.

A vessel must be equipped with: (i) An approved 160–B semi-portable fire-extinguishing system to protect the engine room. Vessels 160B FE pressure gauge was indicating empty.

There must be an installed or portable bilge pump for emergency dewatering. Vessel had one pump not operating properly and the other failed to energize.

Each life float and rigid buoyant apparatus must be fitted with a lifeline, pendants, a painter, and floating electric water light approved under approval series 161.010. Vessels waterlight battery was found to be expired.

Each lifejacket must be marked: (1) In block capital letters with the name of the vessel. Vessel had multiple LJ's without the vsl name stenciled on.

The owner, managing operator, or master of a towing vessel must ensure that the vessel’s lifesaving and fire suppression and detection equipment and is in good working order. Vessels fire nozzle had no gasket and was leaking excessively from the connection point.

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. Vessels charts and pubs were not up to date.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. Vsl's smoke detector in the galley failed to operate.

Electrical components that pose an electrical hazard must be in an enclosure. Breaker panel in the eng. rm. had a cover that was not properly secured.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Identified a dead end wire above the port MDE and a grounding strap on the stbd side switchboard that was not properly grounded.

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The fuel hose on the generator after the filter was found to be dry rotted.

For towing vessels with berthing aboard, a sufficient number of additional lifejackets must be carried so that a lifejacket is immediately available for persons at each normally manned watch station. Vessel only had 3 lifejackets onboard. A total of 6 lifejackets shall be onboard, one placed on the bridge and another in the engine room.

A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 of § 141.370 for specified areas where the vessel operates. Vessel is required to have 6 red hand held flares and 6 orange flares for a total of 12 flares.Vessel only had a total of 6 distress signals.

Survival craft and stowage locations must be marked in accordance with the requirements of §§ 199.176 and 199.178 of this chapter. Retro-reflective tape on life float needs to be replaced.

Distress signals must be stowed in a properly marked portable watertight container at the operating station. Distress signals were not stowed in a container. Properly stow distress signals.

Each life float and ridgid buoyant apparatus must be fitted with a lifeline, pendants, painter, and floating electric waterlight. Vessel survival craft did not have a painter.

Vessel survival craft had holes, and is in need of repair.

Marine employer must have sufficient number of alcohol testing devices onboard. Vessel did not have testing devices onboard to be completed within minimum time frame.

Fuel shut off valves were not labled, nor had direction of proper use.

20-13-NA-13199- Relief valve on pressure vessel capacity is unknown, last testing is also unknown. Pressure vessel was not secured to deck or bulkhead. 46 CFR 143.300(a)

An existing towing vessel subject to this subchapter will remain subject to Coast Guard regulations applicable to the vessel on July 19, 2016 until either July 20, 2018 or the date the vessel obtains a COI, whichever date is earlier. Due to the company not meeting the requirements of 46 CFR 136.202(a)(2), Marine Inspectors attended vessel to ensure compliance with Subchapter M. Upon attendance attending MI's discovered objective evidence that the vessel is not in compliance with Subchapter M. Vessel is required to meet one of the following: either have the vessel surveyed in accordance with 46 CFR parts 137.215 and 137.220 by a qualified surveyor as stated in 46 CFR 139.130 OR an inspection completed by the CG. The vessel will be required to have a COI prior to operating.
