To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be installed and be operable from a safe place outside the space where the valve is installed. The remote fuel shutoff was inoperable. Repair remote fuel shutoff.

Fuel hose, if designed for use with clamps, must be installed with two hose clamps at each end of the hose. Some of the fuel hoses over barbed fittings on the generators were not double-hose-clamped. Install additional hose clamps on all fuel hoses or otherwise make hoses comply with 46 CFR 143.265(d)(2)(v).

Piping for flammable/combustible liquid, seawater cooling, or firefighting systems must be fitted with valves that are clearly marked by labeling or color coding that enables the crew to identify its function. Color code or label applicable piping in engine room.

Each towing vessel must carry at least one appropriately-sized life jacket (meeting approval series) for each person onboard, with additional life jackets available at each watch station. Vessel was missing 01 life jacket required by the vessel's COI. Acquire and properly outfit missing life jacket.

Each inland towing vessel must be equipped with navigation lights in accordance with 33 CFR part 84. Stern navigation light was inoperable. Repair light.

Each hose assembly used for transferring oil must be tested annually to 1.5 times max allowable working pressure and must be marked with test pressure and test date. Ensure fuel transfer hose is properly pressure tested and labeled as described above.

Repairs and minor alterations to existing towing vessels that are not "replacements-in-kind" must comply with the requirements of Subpart C of Part 143. As per 46 CFR 143.510, verification of compliance with machinery design standards is obtained by following provisions in 46 CFR 144.135 through 144.145. As per 46 CFR 144.135, new installations must undergo verification before a new installation is performed. New engines that were installed onboard vessel did not undergo design verification prior to installation. Ensure design verification is completed for the new engine installation by a registered professional engineer or an authorized class society (see 46 CFR 144.140).

Electrical systems and equipment must minimize system failures and fire and shock hazards. Electrical systems must be suitably installed to prevent them from coming loose through vibration. Electrical meters on generator and engine room light fixture have vibrated loose from their permanent mounts, causing an electrical hazard. Properly mount meters and light fixture.

M&P Barge was not conducting drug testing background checks in accordance with 49 CFR 40.25.

M&P Barge had no documentation that current employees has proper EAP training.

M&P Barge did not have documentation that supervisory EAP training was conducted.

Random drug screening was conducted last year, but it was not evenly spread throughout the year. All randoms were administered at the end of the year between November and December of 2012.
