A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturer's instructions. Observed the overriding function of the Fireboy system was unable to be properly function and restart the engine in a timely manner. (46 CFR 181.420(a)(4))

Each lifesaving appliance must be in good working order and ready for immediate use at all times when the unit is in operation. Observed the ring life buoy needs to be replaced due to peeling of outer protecting coating. (46 CFR 180.70(b)(2))

Each lifesaving appliance must be in good working order and ready for immediate use at all times when the unit is in operation. Observed Jim buoy had cracks on its surface and small chunks missing from the body. (46 CFR 109.301(a))

Electrical equipment exposed to corrosive environments must be of suitable construction and corrosion-resistant. Observed one of four batteries improperly secured allowing part of the battery to lay in the corner hull of the bilge. (46 CFR 183.210(c))

The design, materials, and construction of masts, posts, yards, booms, bowsprits, and standing rigging on a sailing vessel must be suitable for the intended service. The hull structure must be adequately reinforced to ensure sufficient strength and resistance to plate buckling. MI observed missing cotter pin on the turnbuckle holding the deck partner in place, new cotter pin has been installed. (46 CFR 177.330)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel has suspected internal structural damage to FWD bulkhead following a collision. Complete inspection of damaged area with marine inspector prior to carrying passengers.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Operator was not able to provide a fire servicing report for the installed system.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel was missing one bucket.

Each survival craft must be: Stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. Vessel is required to use a Hydrostatic Release Unit (HRU) for the Lifefloat. The HRU has expired.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: (1) Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. None of the crew held such license. Provide proof of license for all crew that company will employ as radio operator.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: (1) The pump is listed by an independent laboratory as meeting the requirements in UL 1113 (incorporated by reference; see 46 CFR 175.600). (Bilge pump onboard is not UL 1113. Additionally, pump seems to run incessantly which is a concern for overheating/fire). (3) The pump is permanently mounted. (Pump is not permanently mounted and sloshes around in the bilge). (7) A positive shutoff valve is installed at the hull penetration. (There is no positive shutoff valve at the hull penetration).

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Marine Inspector found that operator is not following the approved Preventative Maintenance Plan for sail rigging onboard this vessel and has made pen and ink changes that required specific inspections by crew. As a result and as required by NVIC 02-16, Marine Inspector found several areas that require inspection by a Third Party Rigging Surveyor: -All rigging aloft (No aloft rigging inspection has been done in last year). -Inspect forestay turnbuckle under roller furling drum. -Inspect backing plate at very bottom of forestay chainplate. Inspector found wasted backing plate beyond repairable scantlings. -Inspect pitting and corrosion in mast step. Rigging Surveyor shall be vetted IAW NVIC 02-16 by OCMI prior to inspection and be able to provide a written report of findings and recommendations. Marine Inspector shall attend Third Party Rigging Survey.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and General description of the drill scenario and training topics. Vessel has not logged any drills for the year 2024 and Master has not participated in a drill since hire. Marine Inspector conducted drills with crew and ensured that was entered in logbook.

Electrical equipment on a vessel must be installed and maintained to: Provide services necessary for safety under normal and emergency conditions; Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; Minimize accidental personnel contact with energized parts; and Prevent electrical ignition of flammable vapors. A wire was disconnected and not properly terminated. Repair to the satisfaction of the marine inspector.

A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Operator could not provide proof of medical certificate. Provide proof to the satisfaction of the marine inspector.

The control cabinets or spaces containing valves, manifolds or controls for the various fire extinguishing systems must be marked in conspicuous red letters at least 2 inches high: “[STEAM/CARBON DIOXIDE/CLEAN AGENT/FOAM/WATER SPRAY - as appropriate] FIRE APPARATUS.” Remote fireboy activation did not have a sign. Create proper signage to the satisfaction of the marine inspector.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit had expired burn gel and eye ointment. Replace to the satisfaction of the marine inspector.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Vessel's rigging plan and maintenance system did not match recommended manufacturer's instructions. Operator updated plan.

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free. Life float is stowed with a cover that will not allow device to float free. Updated 04MAR2022 to authorize one time charter before installing HRU.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section. The owner must provide satisfactory evidence of the required servicing to the marine inspector. Owner did not have annual service certificate from servicing company for pre-engineered firefighting system.

A sailing vessel underway shall exhibit sidelights and a sternlight. Starboard running light is extinguished.

Fuel shutoff valves must be fitted in the fuel supply lines to stop fuel slow. Emergency fuel shutoff line needs wooden cover installed at all times in order to actuate valve.

A VESSEL MUST COMPLY WITH THE APPLICABLE REQUIREMENTS FOR ANY RADIO AND EPIRB INSTALLATIONS, INCLUDING THE REQUIREMENTS FOR A STATION LICENSE AND INSTALLATION CERTIFICATES TO BE ISSUED BY THE FCC, AS SET FORTH IN 47 CFR 80. DISCOVERED VESSEL'S FCC STATION LICENSE WAS EXPIRED. VESSEL RESTRICTED TO VOYAGES LESS THAN 1000FT FROM SHORE. 46 CFR 184.502

EACH LIFE FLOAT MUST BE MARKED OR PAINTED WITH THE VESSEL'S NAME AND TOTAL NUMBER OF PERSONS CAPACITY OF EACH SPECIFIC UNIT IN CLEARLY LEGIBLE LETTERS AND NUMBERS. LIFE FLOAT ONBOARD WAS MISSING THE REQUIRED MARKINGS. 46 CFR 185.604(a) & 46 CFR 185.604(e)

A SHIP MUST HAVE AN OIL POLLUTION PLACARD OF AT LEAST 5 BY 8 INCHES, MADE OF DURABLE MATERIAL FIXED IN A CONSPICUOUS PLACE IN EACH MACHINERY SPACE, OR AT THE BILGE AND BALLAST PUMP CONTROL STATION. VESSEL WAS MISSING AN OIL POLLUTION PLACARD. 33 CFR 155.450

WIRES AND CABLES SHOULD BE INSTALLED IN AN ORGANIZED MANNER WITH METAL SUPPORTS SPACED NOT MORE THAN 610 MILLIMETERS (24 INCHES) APART. WIRES NOT IN USE SHALL BE CAPPED TO REDUCE A POTENTIAL HAZARD. MULTIPLE EXPOSED WIRE ENDS DISCOVERED, ALONG WITH INSUFFICIENT SUPPORTS FOR THE WIRES AND CABLES ONBOARD. 46 CFR 183.340

EACH CHILD SIZE LIFE JACKET MUST BE STOWED IN A LOCATION THAT IS APPROPRIATELY MARKED AND SEPARATED FROM ADULT LIFE JACKETS SO THE CHILD SIZE LIFE JACKETS ARE NOT MISTAKEN FOR ADULT LIFE JACKETS. CHILD SIZE LIFE JACKETS WERE NOT SEPARATED FROM THE VESSEL'S ADULT SIZE LIFE JACKETS. 46 CFR 180.78(a)(5)

Inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails;Vessels forward headstay has a crack on the left side where the member meets the main deck. Provide proof of the extent of the crack and repair the crack.

Repairs or alterations to the hull, machinery or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel reported damage to the bow of the BAY WOLF. Vessel must make repairs to the satisfaction of the OCMI prior to carrying passengers.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from machinery failure. The vessel's propeller is not operating as intended in astern operation. Provide technician's feedback on current propeller operation. Submit repair proposal to satisfaction of the Marine Inspector. 46 CFR 182.100

At each initial and subsequent inspection for certification, the owner shall be prepared to have the vessel ready for inspection. A log of the inspections and maintenance shall be maintained by the Captains and Crew of the vessel and shall be kept on board for review as requested by USCG Marine Safety Inspectors. Maintenance records not available for review. Provide maintenance records following sailboat specific preventative maintenance schedule. 46 CFR 176.802(a) NVIC 02-16

All cable and wire must be installed with metal supports spaced not more than 24 inches apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations. Numerous runs of cable throughout the vessel are lacking metal supports. Install metal supports spaced not more than 24 inches apart IAW 46 CFR 183.340. Provide proof of installation to satisfaction of Marine Inspector. 46 CFR 183.340(b)(4)

All cable and wire must be installed with metal supports and in such a manner as to avoid chafing and other damage. The forward electric windlass control is being supported by rope. Install metal support IAW 46 CFR 183.340. Provide proof of installation to satisfaction of Marine Inspector. 46 CFR 183.340(b)(4)

Each hatch used as a means of escape must have a permanently installed handle not capable of being removed. The forward escape hatch is opened with a wrench temporarily installed. Permanently install handle IAW 46 CFR 177.500. Provide proof of installation to satisfaction of Marine Inspector. 46 CFR 177.500

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present such as machinery spaces. The Engine Compartment does not have a suitable vapor barrier. Repair vapor barrier IAW 46 CFR 177.405. Provide proof of repair to satisfaction of the MI. 46 CFR 177.405(e)

The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. The fuel supply lines for the propulsion engine are attached to the bulkhead with plastic zip ties. Renew the plastic supports with metal supports to offer continued support in the event of a fire. Provide proof of repair to satisfaction of Marine Inspector. 46 CFR 177.405(a)

The weather deck on a flush deck vessel must be watertight and have no obstruction to overboard drainage. The vessel's cockpit has drilled holes compromising the watertight integrity of the deck. Repair the deck IAW 46 CFR 178.410. Provide proof of repair to satisfaction of Marine Inspector. 46 CFR 178.410(a)

Deck rails must be constructed to withstand a point load of 200 pounds applied at any point in any direction. The port deck rail has a broken stanchion. Replace during the next dry dock IAW 46 CFR 177.900. 46 CFR 177.900(b)

Make fwd emergency escape hatch operable from both sides.

Provide fireboy operating instruction for engine restart in over-ride mode (post IVO fireboy controls).
