The company should establish procedures to ensure the ship is maintained in conformity with the provisions of the relevant rules and regulations. The vessel’s crew was unable to provide SMS procedures upon request and were unfamiliar with the vessel’s Safety Management System. Based on this and the technical and operational nature of the deficiencies noted below provide objective evidence of a serious failure of implementation of the ISM code. Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the Oil Record Book Part I, so that all entries in the book appropriate to that operation are completed. Each completed operation shall be signed by the officer or officers in charge of the operations concerned and each completed page or group of electronic entries shall be signed by the master of ship. The master of the vessel had not signed required pages and C/E had failed to log the OWS testing. Testing was recorded in a separate binder. MARPOL I/17.4

A line-throwing appliance complying with the requirements of section 7.1 of the Code shall be provided. 04 line-throwing appliances were expired. Vessel showed me a requisition dated 22Jul2024 for new line-throwing appliances, as of inspection dated company had not purchased. SOLAS (2020) III/18

At least 3 two-way VHF radiotelephone apparatus shall be provided on every passenger ship and on every cargo ship of 500 gross tonnage and upwards. 03 batteries for the VHF radiotelephone were expired,(June2024), requistion was submitted 22Jul2024 to replace expired batteries, as of date of inspection company had not purchased. SOLAS 74 (2020) III / 6.2.1.1

In cargo ships of 2,000 gross tonnage and upwards, if a fire in any one compartment could put all the pumps out of action there shall be an alternative means consisting of a fixed independently driven emergency pump which shall be capable of supplying two jets of water to the satisfaction of the Administration. Emergency fire pump was not able to supply the stbd side bridge wing and port side main deck to adequate water. 74 SOLAS (2001) II-2 / 4 / 3.3.2

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Dead end and exposed wires were found in several areas of the engine room. 1. Dead end wire in Ships Alarm Panel 2.Exposed wires in Sound Power Phone Junction Box 3. Emergency lighting at stbd hatch to Z-drive room 4. Shore tie cable stbd side termination into panel 5. Exposed wires next to 20 AMP battery charger in emergency generator space. 74 SOLAS (2001) II-1/45/1.3

Where "A" class divisions are penetrated for the passage of electric cables, pipes, trunks, ducts, etc., or for girders, beams or other structural members, arrangements shall be made to ensure that the fire resistance is not impaired, subject to the provisions of regulation 30.5. Vessel had a hole cut into the stbd side bulkhead from the escape truck to run the shore tie cable into the space. Spray foam was used to seal the area around the cable. The upper cable penetration was missing a portion of the "rocks block" allowing daylight from the open escape hatch to enter the space. 74 SOLAS (2001) II-2/18/1.1

Tie in portable quarters fire alarm to general alarm bell; two minute as per SOLAS requirements; prior to sailing international.

Replace SOLAS decals on bridge wings; distress signals, life rafts, line throwing gun...

Label deep fat fryer and stove emergency disconnects.

Stencil bow thruster vents with proper marking for closure in case of fire.

Provide GMDSS Maintenance endorsement or GMDSS maintenance agreement contract with servicing company.

Provide vessel response plan prior to carring oil base muds or prior to resolution date whichever is first.

Provide approved ABS electrical load analysis.

Prove proper operation of all generator overspeeds prior to getting underway.

Repair marine cable transits in Z drive rooms to engine room prior to getting underway. Transits are miss matched with two different vendors.

Replace gasket on water tight door to thruster room.

Repair fuel oil leak on center generator engine prior to getting underway.

Provide proposal for repair or equivalent safety to water tight door opening into CO2 protected bow thruster room.

Demonstrate Proper Operation of WTD leading to Bow Thruster room. Door does not close.

Provide Auto door closure for all WTDs that surround CO2 protected space.

Complete annual service for fixed CO2.

Demonstrate proper operation of SSB on emergency power

Provide emergency power for extra VHF radio.

Provide Visual and Audible alarm for engine room bilge alarm at operating station.

Post General Alarm placards at all GE bells, including the ROV spaces.

Update call sign onboard in accordance with SSL. Emergency Boradcast Placard, EPIRB registration, AIS etc.

Provide first aid kit that does not have expired medicine.

Log Rescue Boat operation.

Provide rubber mat in front of emergency switchboard.

Fix small f/o leak in bow thruster f/o filter and sight glass.

Label emergency steering station P/S.

Demonstrate Proper Operation of all three generator overspeed trips.

Label bilge manifold.

Permanently wire magnetic pick-up on stbd generator - no wire nuts.

Provide Fire and Safety Plan for Approval

Provide/Post Waste Management Plan

Install fire pump gauge at operating station.

Prior to sailing, provide suitable wrenches and prove proper operation of all fuel oil cutout valves/reachrods on cargo deck.

Provide approved drawings/ manufactures wiring specifications to resolve tripping of bow thruster hydraulic pumps when ventilation shutdown is activated.

FUEL OIL TRANSFER PROCEDURES MUST BE SUBMITTED TO MSO MOBILE FOR REVIEW IAW33CFR 155.720.

PROVIDE COPY OF PERMANENT RADIO STATION LICENSE TO MSO MOBILE.
