Attending marine inspector observed repairs made to both aluminum conduits/ support structures identified in Deficiency # 7 of this inspection without proper approval. The outboard portion (from 2 o'clock to 6 o'clock when observed from the forward entry to the tank space on the port side unit) of the weld presents with porosity and signs of corrosion on the base material. Remove suspect portion of the weld to expose base material for further evaluation of repairs as made. 46 CFR 176.610(b)

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. Stbd quarter rail station separated from base. Repair in accordance with procedure used to correct the 02 other separated stantions.

Branch circuit conductors for hot water supply boilers which have a capacity not greater than 454 liters (120 U.S. gallons) must have a current carrying capacity of not less than 125 percent of the current rating of the appliance. No appropriate overcurrent protection installed for water heater. Install appropriate over-current protection IAW email correspondence dtd. 08 May 2026. 46 CFR 63.25-3(c)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Battery charger lacks overcurrent protection as required per manufacturer specification. Install appropriate over-current protection IAW email correspondence dtd. 08 May 2026.

Each circuit breaker must meet UL 489 (incorporated by reference; see 46 CFR 175.600) or other standard specified by the Commandant, and be of the manually reset type... Multiple breakers in the white housing on forward engineroom bulkhead and on stbd bulkhead of teh ladder well to heads not labeled as UL compliant. Install appropriate over-current protection IAW email correspondence dtd. 08 May 2026.

ll cable and wire must... Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Wiring throughout fuel tank compartment supported by zip-ties. Replace with approved securing devices.

A drydock examination conducted in compliance with § 176.600 must be conducted while the vessel is hauled out of the water or placed in a drydock or slipway. During the examination all accessible parts of the vessel's underwater body... Damage identified on stbd sheerstrake and stbd quarter under exhaust discharge. Expose to good material and provide repair proposal to the satisfaction of the attending marine inspector prior to credit drydock.

Each item of lifesaving equipment determined by the Marine Inspector to not be in a serviceable condition must be repaired or replaced. Vessel's owner informed Marine Inspector that both lifefloats were not in serviceable condition from lack of maintenance. The attending Marine inspector(s) confirmed that the lifefloats were deteriorated beyond the point of serviceability and that float free arrangements were not maintained to function as designed. Vessel Owner has requested to limit the vessels route on Certificate of Inspection due to being in compliance with §180.205(e)(3); having on board a FCC Type Accepted Category 1 406 MHz EPIRB installed to automatically float free and activate. Effective immediately, the vessel may not operate in waters beyond 3NM from land. (1) Provide written request to OCMI to remove lifeloats from vessel or (2) Repair/replace lifefloats and float free arrangements to the satisfaction by the attending Marine Inspector. 46 CFR 176.808(b)

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. Starboard forward bulkhead in engine space contained an opening that compromised vapor tight requirement. Fill/plug opening to the satisfaction of the attending Marine Inspector. 46 CFR 177.405(e)

An EAP training program must be conducted for the employer's crew members and supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Vessel did not provide documentary evidence that crew and supervisory personnel received required training. Provide a written record of crew and supervisory training conducted for vessel personnel to the satisfaction of the attending Marine Inspector. 46 CFR 16.401(b)

Each ring life buoy on a vessel must: (1) be approved un accordance with 46 cfr 160.050 in subchapter Q of this chapter... Ring life buoy has numerous tears and grab line was dry rotted. 46CFR180.70 (b)(1)

Remote fuel shutoffs stations must be marked in clearly legible letters... Fuel shutoffs markings were faded away. 46CFR185.608

The area of operation for each vessel and any necessary operational limits are determined by the cognizant OCMI, and recorded on the vessel's Certificate of Inspection. The vessel's ownership has recently changed resulting in the vessel moving from Maui to Oahu to conduct commercial passenger operations. Appropriate documentation has been applied for to reflect the ownership change; however, NVDC has not processed yet. The vessel's new route is: PACIFIC OCEAN, STATE OF HAWAII, SOUTHERN AND WESTERN COASTS OF THE ISLAND OF OAHU BETWEEN A LINE EXTENDING SOUTH BY SOUTHWEST FROM KOKO HEAD TO A LINE EXTENDING SOUTHWEST FROM KAENA POINT, NOT MORE THAN 20 MILES FROM A HARBOR OF SAFE REFUGE. All other conditions of operation listed on the COI remain applicable and in effect. COI will be amended upon owner's receipt of updated COD. 46 CFR 176.110(a)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its COI giving consideration to provisions for a seaworthy hull. Starboard side hull mid ship suffered damage approximately 24 x 4 inches in size above the water line. Vessel allided with pier. Make permanent FRP repairs in accordance with NVIC 8-87 prior to taking passengers. 46 CFR 177.100

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed mush have the following equipment at the operating station: A means to reset devices used to automatically shutdown ventilation systems and engines as required by paragraph (a)(3) of this section. The Fire boy override was inoperative during testing. 46CFR181.420(b)

The painter must: Have breaking strength of at least 1,500 pounds. The painter line for the life float was wasted. 46CFR180.175(3)(ii)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of Large enough scale to make safe navigation possible. Chart on board where not up-to-date. 46CFR184.420(a)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel's life ring buoy is worn. Replace life ring buoy. 46 CFR 176.808(b)

Each paddle required for life floats must be at least four feet in length, lashed to the life float to which it belongs and buoyant. Life float paddles are less than 4 feet and are not lashed to the life floats. Provide the appropriate number of paddles properly arranged to life floats on board vessel. 46 CFR 180.175

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and Supervisory personnel must receive at least 60 minutes of training. Provide proof of completed EAP training for all personnel.

01 bilge high level visual alarm indicator not operational. Visual and audible indicator must be provided at operating station. Provide operational visual alarm indicator.

01 automatic bilge pump indicator not operational. Visual indicator must be provided at the operating station for any automatic bilge pump when operating.

Life float light housing cracked. Provide new floating water light for life float. Prior to nighttime operations.

Ring buoy floating water light not operational. Make ring buoy floating water light operational. Prior to nighttime operations.

Navigation publications not on vessel. Provide navigation publications.

Engine compartment Fireboy audible alarm did not sound when tested. Prove correct operation in accordance with 46 CFR 181.420.

Lifefloat arrangement was unsatisfactory. Lifefloats utilized single painter and were not chained together. Painter was made to grab line of single lifefloat. Painter was detriorated. Prove correct lifefloat arrangement in accordance with 46 CFR 180.137.
