A vessel of 12 meters or more in length shall be provided with a whistle. Test found solenoid valve inoperable. 33 CFR 83.33(a)

The inspector shall examine all accessible parts of the vessel's hull,machinery, and equipment to be assured that it is in a satisfactory condition. Forward pilothouse bulkhead at main deck contained a cracked weld allowing water intrusion into fiddley space. 46 CFR 91.27-5

A rescue boat and its installation shall be acceptable to the OCMI for the rescue of persons accidentally falling over the side. Nylon lifting straps were worn and original material supplied with rescue boat in 2019. The straps shall have a design safety factor of 6 times the max working load. 46 CFR 94.10-10(e), District 8 Rescue Boat Policy Nov 12,1998

Cranes and other hoisting machines with their accessory gear, at least once in each five years, shall be tested with a proof load which shall exceed the safe working load as set forth in Table 91.37-40(a).If the boom of the crane has a movable radius, it shall be tested with a proof load as set forth in this section at the maximum and minimum radius of the boom. Port side Nautilus crane is rated for 26.8 tons safe working load and was initially load tested on 9/06/22, but did not achieve 5 ton in excess due to wire rope coming out of sheave groove. Vessel completed a follow up test at 25% (2,300 lbs) in excess that is reserved for cargo gear not exceeding 20 tons. Test was only completed at one angle. Starboard EBI crane with a safe working load of 9.7 tons was tested at the required 25% excess, but only at one angle. 46 CFR Subchapter Old I, 46 CFR 91.37-3(a), 46 CFR 91.37-40(a), 46 CFR 91.37-40(d).

Pressure vessel (PV) gauges shall be no less than 1.2 times nor more than 2 times the pressure at which the pressure relief valve is set. The PV gauge was inoperable & the relief valve did not have a tag to determine its actual setting. Vessel shall replace/repair gauge & identify/mark pressure relief valve setting prior to getting underway. 46 CFR 54.15-5(f)

General alarms shall be supplemented with a flashing red light in locations where noise levels are too high to hear the audible alarm. The port side engine room general alarm flashing red light stopped working during the inspection. Vessel shall replace/repair the flashing red light prior to getting underway. 46 CFR 113.25-10(d)(3)

Emergency lighting shall be operated & inspected at least once a week while the vessel is underway. The emergency light in berthing area "A" was inoperable & the emergency light in the starboard engine room was too dim to provide adequate lighting for egress. Vessel shall replace/repair the emergency lights prior to getting underway. 46 CFR 97.15-30(a)

Means shall be provided to close of all vents. A few of the vents on the main deck were unable to be closed due to broken/missing closure devices. Vessel shall ensure all vent closures are in working order prior to getting underway. 46 CFR 92.15-10(a)

Lifefloats shall be marked with the vessel's name & number of persons allowed. The port lifefloat's markings were faded & becoming unreadable. Vessel shall re-mark the required markings on the lifefloats. 46 CFR 97.37-40(a) 46 CFR 97.37-40(b)

At each inspection for certification the inspector shall determine that all fire fighting equipment is in suitable condition. The main deck fire axe blade is wasted. Vessel shall replace fire axe. 46 CFR 91.25-20(a)

Overspeed trips were not calibrated or installed for port and starboard generators.

Vessel did not have L/O analysis completed for 5 yr Leg credit IAW CG-543 Policy Ltr.

2 BII portable extinguishers in Galley were found not annually inspected. Crew to have 3rd party provide inspection.

Lifefloat painter line was worn.

Rescue boat contains thru hull fasteners that lifting straps attach to place boat in water. Two fasteners were cut short causing locking nuts not to be installed correctly. Crew to replace.

Draft and Loadline marks are not visible and need to be repainted in contrasting color to the hull.

#1 Steering power failure alarm did not work properly and needs to be fixed.

The two lifefloats were not attached to painter correctly. Painter was intended to be linked in series, however one lifefloat was not actually attached and must be corrected.

Exhaust hangers for port engine exhaust has cracked welds and needs to be repaired.
