The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all aspects will remain fit to proceed to sea without danger to the ship or persons onboard. Vessel reported damage to one of the blades on the port controllable pitch propeller. SOLAS 74 I/II (a) (2020 Con. Ed.)

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. VSL reported to NRC that Liberty Island lost 27.5 gallon of CPP fluid/oil in the last 24hrs. Vsl inspected the vessel for the loss and assumed its in the water by way of CPP blade seals. VSL is to cease operation immediately, transit directly to Big Stone Anchorage where the VSL will await ABS and a dive survey team to further assess the CPP casualty/leak. Once at Big Stone Anchorage VSL is restricted prior to operation and restricted prior to departure until satisfactory testing and operation are conducted by ABS.

The machinery associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. MI found excessive leaking coming from STBD dredge pump where the clean out mounts to the pump. Make repairs to the satisfaction of the TPO(ABS). 74SOLAS(04)II-1/26.1

Recieved notification that vessel is having difficulties with port CPP controls. Repair CPP to satisfaction of attending ABS Class Surveyor and USCG Marine Inspector prior to movement. 46 CFR 91.30-1

74 SOLAS (04 CONS) II-1/41.1.1 A main source of electrical power of sufficient capacity to supply all those services mentioned in 40.1.1 shall be provided. This main source of electrical power shall consist of at least two generating sets. STBD generator is unable to supply electrical power to ship services.

Arrangements for oil fuel, lubricating oil, and other flammable oils. (MSC/Circ. 647): 4.3 Any fuel, lubricating or hydraulic oil leakages should be dealt with promptly. The screening arrangements and pipe securing devices should be kept in good order. Marine Inspector found substantial accumulation of leaked/leaking hydraulic oil in the hydraulic room coming from various piping and associated equipment. Additionally, identified high pressure pipe approximately 3’- 4’ for the portside dragnet that had been repaired twice in August 2023 that was still leaking. 74 SOLAS (01) II-2/Reg 15, MSC/Circ. 647 Code 705: 30 days

Marine Inspector identified approximately 5” piping vent for the evaporator booster pump located in the forward pump room that has severe wastage with a temporary patch that has been installed previously. The temporary patch is leaking through. From the booster pump to affected area is approximately 20’- 30’ up. 74 SOLAS (04) II-1/Reg. 26.1 Code 705: 30 days

Every oil fuel pipe, which, if damaged, would allow oil to escape from a storage, settling or daily service tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. Marine Inspector witnessed Chief Engineer test the emergency fuel quick closing valve for the Emergency Diesel Generator tank. Found valve not activating/securing when actuated outside space. Identified gate valve was closed not allowing the valve to operate properly. Opened gate valve manually and tested emergency shutdown again with satisfactory results. The emergency shut-down was previously tested when PSTP’s were conducted March 2022 – March 2023. 74 SOLAS (01) II-2/Reg. 15 2.5

Stairways which penetrate more than a single deck shall be surrounded by “A-0” Class divisions and be protected by self-closing doors at all levels. Marine Inspector found the main deck & foscle deck stair tower fire doors not closing all the way. Crew adjusted both doors correcting them on the spot. 74 SOLAS (01) II-2/Reg 46.1

Access doors and access hatch covers normally closed at sea, intended to endure the watertight integrity of internal openings, shall be provided with means of indication locally and on the bridge showing whether these doors or hatch covers are open or closed. Marine Inspector witnessed the operation of the aft steering watertight hatch was secured properly, found indicator light solenoid affixed to the bulkhead was not operating as intended giving indicator lights to the navigation bridge. Solenoid roller indicator were cleaned due to paint build up. Conducted additional operation test with satisfactory results. Deficiency was corrected on the spot. 74 SOLAS (04) II-1/25-9.4

Launching and embarkation appliances complying with the requirements of section 6.1 of the code(LSA) shall be provided for all survival craft. The launching mechanism shall be so arranged so that it may be actuated by one person from a position within the survival craft. BOTH Port & STBD side remote release cable would not actuate the gravity brake from within the lifeboat rendering the lifeboat unable to launch in an emergency abandonment situation.

Launching and embarkation appliances complying with the requirements of section 6.1 of the code(LSA) shall be provided for all survival craft. Manual brakes shall be so arranged that the brake is always applied unless the operator, either on deck or in the survival craft holds the brake in the "off" position. BOTH Port and STBD survival craft brakes would not operate when released by the operator.

Fire-fighting appliances shall be kept in good working order and readily available for immediate use. 01 fire hose in engine room was found with a hole in the hose.

The letters of the load lines assigned shall be painted white or yellow on a dark ground or in black on a light ground and shall be plainly visible. BOTH Port & STBD side load lines were missing the painted markings as follows: TF, F, T, S, W, WNA as shown on the Load Line certificate.

Every ship shall be provided with a main steering gear and an auxiliary steering gear to the satisfaction of the Administration. Vessel experienced an un-commanded variation of 10 degrees of the starboard rudder. Repair/replace main steering gear to the satisfaction of the RO and provide statement from the RO attesting to the repairs and testing of the system, or provide class report of deficiency acceptance to assign as condition of class.

Starboard tailshaft bearing oil seal failed while dredging. Lube oil line and starboard main engine must be secured until replacement. Starboard shaft is to be used on a limited basis.

Provide proof of FCC servicing IAW 47CFR80.59.

Provide 09 Lifeboat food rations IAW 46CFR159.529.

Demonstrate proper operation of watertight door indicators on bridge for hydraulic pump room, jet pump room, and WTD IVO FR 92.

Provide DAPI enrollment crew list IAW 46CFR16.500.

Make permenant repairs to hole in the hull located alongside the port bilge holding tank.

Port MDE Turbocharger bearing failed.

Repair leaking flange on the sea water side of sea chest to the satisfaction of ABS.

Vessel struck submerged object; hull was holed and sideshell damaged; repair as original.

Vessel's Master to submit a sail plan from Beaufort NC to Charleston SC due to damage from striking a submerged object to the number one void/holing vessel. Plan to include: time of departure, time of arrival, drydock facility, and additional safety items in place for the transit.

The Vessel while operating in the waters of MHC, NC struck an unknown submerged object. Vessel examined by a commercial diver; the stbd side shell plating was found heavily set in and hold approx. 24" x 30" in way of thruster tunnel and No. 1 void at frames 131 and 132 with the rest of the stbd SS scraped lightly. Make permenant repairs to the effective area to the satisfaction of ABS/USCG.

Provide a vessel specific Ballast Water Management Plan that will allow those responsible for the plan's implementation to understand and follow the plan as required by regulation.

Provide records for review and ensure all records will be kept on board for the duration of 2 years as required by regulations.

Provide evidence of mandatory ballast water management reporting criteria as required by regulations.

Engine exhibited high exhaust stack temperatures due to leaks at air box.

Submit repair proposal to OCMI, and execute repairs to damaged sideshell and associated structural members IWO #12 void and aft dirty oil tank, stbd side. Prove welds to the satisfaction of attending marine inspector.

Provide reliefs for the 3rd asst. eng and the oiler or provide original STCW certificates for both crew members.

Adjust watertight doors Portside # 68 and Starboard # 43 to prevent the doors from sticking closed.
