Each towing vessel must be fitted with a general alarm that: Is capable of notifying persons in any accommodation, work space, and the engine room. Work space on main deck does not have an installed general alarm. Vessel is to install general alarm that is audible and visual in work space with proper signage to the satisfaction of Marine Inspector.

Each operator or agent of a vessel with a capacity of 250 or more barrels of fuel oil, cargo oil, hazardous material, or liquefied gas as regulated in Table 4 of 46 CFR part 154, or each person who arranges for and hires a person to be in charge of a transfer of fuel oil, of a transfer of liquid cargo in bulk, or of cargo-tank cleaning, shall designate, either by name or by position in the crew, the person in charge (PIC) of each transfer to or from the vessel and of each tank-cleaning. Vessel was found to be conducting fuel oil transfers with the dredge OREGON which does not currently comply with required transfer personnel, procedures, equipment, or records. ITV CLACKAMAS is restricted from receiving fuel from the dredge OREGON until the dredge is in compliance with 33 CFR 155 to the Marine Inspectors satisfaction.

If the master or officer in charge of a navigational watch believes it is unsafe for the vessel to proceed, that an operation endangers the vessel or crew, or that an unsafe condition exists, he or she must ensure that adequate corrective action is taken and must not proceed until it is safe to do so. The Flammable locker in the auxiliary machinery space was unsecured. Secure the flammable locker to the satisfaction of the Marine Inspector.

Verification of compliance with the construction and arrangement design standards of this part must be performed according to the following table: If the vessel is— (c) A vessel on which a new installation that is not a “replacement in kind”, then the applicable requirements must be met — before the new installation is performed. Generator was replaced in JAN 23 with "not in kind." Submit electrical drawings and manufacturer data sheet to the Marine Inspector for review.

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. Vessel operator failed to schedule an annual inspection with the Coast Guard prior within period alotted in 46 CFR 137.200. Apply for COI inspection with local OCMI.

Fuel system requirements for towing vessels require filters and/or purifiers. Where filters are used, the fuel filters must be replaced in accordance with manufacturer's requirements. Racor filters are installed with clear plastic sight glass without protective metal flame bowls. Plastic covers only meet the manufacturer's specifications when installed with the metal flame bowls in a marine application. Install flame bowls to meet the requirement. 46CFR143.255

SC01 - Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. The main engine exhaust piping located on the starboard side of the vessel was within reach of personnel in the near vicinity of the handrails and outboard side for transit. Any location that crew could come in contact with exhaust piping must have an appropriate guard or insulation. Complete within 30 days, if unable to complete, notify the MSU.

SC02 - Each lifejacket must be marked in block capital letters with the name of the towing vessel. In addition lifejackets must have a lifejacket light and whistle secured as outlined in the 46 CFR 141.340. All lifejackets did not have any markings or required additional attachment and fittings. Ensure all lifejackets are readily available, properly marked with vessel name and attach a whistle and lifejacket light to all personnel flotation devices.

13199 - Other(machinery) - All pressure vessels shall satisfactorily pass a hydrostatic test. Pressure vessels observed to have internal corrosion. Conduct hydrostatic testing on the two pressure vessels located forward of the engine space in accordance with cited regulations to verify fitness for service prior to vessel movement. 46 CFR 54.10-15(a).

02111 - Beams, frames, floors-corrosion - The vessel shall be maintained so the structure of the vessel does not cause concern regarding structural integrity. Deck support observed in the forward space against the engine room bulkhead was wasted on the bottom. Provide plans for correction and conduct repairs. 46 CFR 144.200.

07106 - Fire detection and alarm system - Fire detection system shall maintain a visual indicator after the alarms have been acknowledged as referenced in letter dated 06July2018, CORR: 2018-0287, as per 46 CFR 142.330.

08105 - Engineer's Alarm - Visible and audible alarms are required at each operating station for auxiliary generator engine low lubricating oil pressure and high cooling water temperature. Vessel did not have audible alrms at the operating station. Required prior to issuance of COI. 46 CFR 143.230 (a)3&4,(b)(1).

08101 - General Alarm - Vessel must be fitted with a general alarm with flashing red light and signage in the engine room. Vessel did not have a general alrm as required. Required prior to issuance of COI. 46 CFR 143.235(b).

02117 - Decks - Corrosion - Watertight decks must be maintained in that condition. Hole in the deck through to the engine room was noted on the aft deck. Rectify prior to issuance of COI. 46 CFR 140.605.

07103 - Division - decks, bulkheads and penetrations - Vessel's Fire supression system should be maintained in good working order. Forwaed bulkhead has multiple openings into the forward space potentially reducing the effectiveness of the fire suppression system in the engine room. Repair/plug hole in the engine room forward bulkhead. Rectify within 30 days. 46 CFR 140.650.

07199 - Other (fire safety) - Flammable products storage cabinet must be secured. Storage cabinet in the engine room was not secured properly to the vessel. Rectify within 30 days. 46 CFR 142.225 (c).

13199- Other (machinery)- Unable to inspect pressure vessels, relief valves, pressure gages due to location in confined space. Make confined space gas free and pressure vessels available for internal inspection. Required prior to issuance of COI.46 CFR 143.300
