Each lifebuoy on a towing vessel must: (4) Be marked in block capital letters with the name of the vessel. Observed Lifebuoys onboard not exhibit vessels name properly. Lifebuoys must be relabeled. 46 CFR 141.360 (b)(4) 16c

All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. Observed no nameplate attached to pressure vessel. Provided permanent attached MAWP display to pressure vessel. 46 CFR 143.300 (c) 16c

Electrical components that pose an electrical hazard must be in an enclosure. Observed exposed electrical wiring in engine room at aft entrance. Secure wiring to prevent accidentally contact. 46 CFR 143.400 (g) 16c

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Observed Engine Room ventilation ducts without a means to secure ventilation. Provide covers for exhaust ducts. 46 CFR 144.605 16c

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and must be either: (2) Non-electric, phosphorescent adhesive lighting strips that are installed along escape routes and sufficiently visible to enable egress with no power. Observed no emergency lighting or lighting tape at either escape routes from engine room. Provide sufficient emergency lighting for space. 46 CFR 143.410 (a)(2) 16c

Propulsion machinery, including main engines, reduction gears, and shafting must be maintained to ensure proper operation. Repairs to shafts and propellers must be made to the satisfaction of the Coast Guard prior to cargo operations. 46 CFR 143.205

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Liferaft bracket, hydro-static release and associated equipment were missing. 46 CFR 141.230 - 701 C

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Marine Inspector observed steering system not functional at the time of inspection. Vessel is required operational steering system for safe operation. 43 CFR 143.205

A sufficient amount of lined commercial fire hose 15 meters (50 feet) in length, at least 40 mm (1.5 inches) in diameter and immediately available to attach to it so that a stream of water will reach any part of the vessel. Marine Inspector observed hose ruptured at base of bracket rendering hose ineffective. Hose requires replacement. 46 CFR 142.325(e)(2)

A remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. Marine Inspector observed improper configuration on the Port side Fuel shutoff value restricting operation of value. Value requires full movement for operation. 46 CFR 143.260 (b)

Each distribution panel or switchboard on a towing vessel must be: Totally enclosed and of the dead-front type. Marine Inspector observed circuit breaker without breaker, covered in tape. Circuit Breaker requires covering from protection of electrical shock and required each breaker to be labeled. 46 CFR 143.560(a) (2) 46 CFR 143.400(d)

Electrical components that pose an electrical hazard must be in an enclosure. Marine Inspector observed a electrical box in engine room bilge with exposed wiring. Electrical box is required to be in a enclosure. 46 CFR 143.400(g)

Pressure vessels must be externally examined annually. Relief valves must be tested in accordance with 46 CFR 143.245. Marine Inspector observed Pressure Vessel relief value with no tag. Relief value needs to be replaced. 46 CFR 143.300(d)

Life jackets must have the following attachments and fittings: (1) Each life jacket must have a life jacket light securely attached to the front shoulder area of the life jacket. (2) Each life jacket must have a whistle firmly secured by a cord to the life jacket. Marine Inspector observe all life jackets with either a Light or Whistle secured to the jacket. All life jackets requires a whistle and light. 46 CFR 141.340(g) (1-2)

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: (a) Fathometer. Marine Inspector observed depth sounder not functional. Fathometer is required to be operational. 46 CFR 140.725(a)

Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. Marine Inspector observed exposed exhaust piping in engine room on both main engines. All exposed exhaust piping is required protection from injury. 46 CFR 144.830

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. Marine Inspector observed loudhailer bracket broken and hanging by electrical wiring in engine room. All electrical components are required to be suitably installed. 46 CFR 143.400(e)

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire. Marine Inspector observed engine room ventilation openings without sufficient means for closing. Ventilation intakes are required a covering to enclosed machinery spaces. 46 CFR 144.605

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. Marine Inspector observed no navigational watch assessments recorded in log nor checklist in accordance with requirements in 46 CFR 140.635. Vessel is required Navigation Assessments prior to getting underway. 46 CFR 140.635(a)

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. Marine Inspector observed no records of drug and alcohol testing records for crew members since 2022 and no on board testing equipment. Vessel is required a drug and alcohol surveillance program. 46 CFR 16.201(a)

Piping and machinery components that exceed 220 degrees Celsius must be insulated. Marine inspector observed torn insulation on the exhaust for the starboard main engine. 46 CFR 143.220(b)

Each life float must be fitted with a floating electric water light approved under approval series 161.010. Marine inspector observed that the life float water light had an expired battery. 46 CFR 141.325(c)

Each life float must be fitted with a painter. Marine inspector observed that the painter was attached the vessel, and would not break free should the vessel sink in water deeper than the length of the painter. 46 CFR 141.325(c) 46 CFR 160.073

Machinery systems must be maintained to provide for safe operation of the towing vessel and safety of persons on board. Marine inspector observed oil build up on the forward side of the generator winding. Recommend Megger Test to confirm generator continuity and assure windings are in satisfactory condition. 46 CFR 143.205(a)

Each towing vessel must be maintained so as to minimize fire hazards to ensure all bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids. Marine inspector observed approximately 2" of oily water built up in the bilge. In addition, there was a build up of hydraulic oil in the cofferdam for the stbd side shaft alley. 46 CFR 142.220(a)

Items recorded as part of the TVR must include the date and time of entry and name of the person making the entry. Marine inspector observed that the vessel records did not indicate a name associated with the conducted checks. 46 CFR 142.220(a)

insofar as structural adequacy is concerned and does not cause the structure of the vessel to be questioned by either the OCMI, or TPO engaged to perform an audit or survey. Marine inspector observed a hole in the starboard aft, ceiling portion causing a risk for down-flooding. 46 CFR 140.915(b)

Multi-outlet adapters (power strips) may not be connected to other adapters (“daisy-chained”), or otherwise used in a manner that could overload the capacity of a receptacle. Marine inspector observed crew rest area with multiple "daisy-chained" adapters in use. 46 CFR 143.575(c)

The cognizant OCMI may require additional examinations of the vessel whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness for the route or service of a vessel. Corroded and holed longitudinal stiffners identified by the MI in the forward void were marked during ISE to be cropped and renewed. 46CFR137.300(b)

Each towing vessel must have a fire-detection system which draws power from two sources. Battery back up did not activate upon loss of power to fire-detection panel. 46CFR142.330(a)(5)

The cognizant OCMI may require additional examinations of the vessel whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness for the route or service of a vessel. Vessel has multiple areas of significant wastage in voids. Clean and gas-free all voids to allow for inspection from attending marine inspector. 46CFR137.300(b)

The owner or managing operator of a towing vessel must establish procedures to address fall overboard prevention and recovery of persons in the water, including, but not limited to: Personal protective equipment, safely working on the tow, safety while line handling, safely moving between the vessel and a tow, pier, structure, or other vessel and use of retrieval equipment. Vessel did not have procedures located on board. 46CFR140.425(a)

The owner or managing operator must implement a health and safety plan. The health and safety plan must document compliance with this part (46CFR140.500) and include recordkeeping procedures. Vessel did not have a Health and Safety Plan. 46CFR140.500(a)

The results of the inspections required by 33CFR164.76 must be documented in the TVR or official logbook. Vessel had no documentation of inspections of any towing gear on board. Chock on port side seized. 46CFR140.820

Each life float and rigid buoyant apparatus must be fitted with a lifeline, pendants, a painter, and floating electric water light approved under approval series 161.010. Life float did not have floating water light. 46CFR141.325(c)

Each vessel operating on a lakes, bays and sounds route no more than 3 miles from shore must have 3 day and 3 night visual distress signals. Vessel missing distress signals. 46CFR141.370

Towing vessels must meet the communications requirements of 33 CFR 164.72 and must be licensed by the FCC. Vessel did not have FCC certificate. 46CFR140.715

Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition. Hole cut into bulkhead on deck, starboard side IVO fire axe. 46CFR140.610(f)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Multiple dead-end wires,blade fuses, and A/C circuit breakers mounted outside junction boxes and electrical panels located below operating station. 46CFR143.400(a)
