In closed RO-RO cargo spaces… ventilation fans shall normally be run continuously whenever vehicles are onboard. All cargo ventilation fans were secured at the request of the stevedores due to them being cold.

The condition of the ship and it’s equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed several areas of inadequate installation causing ice build up around the low pressure CO2 system.

The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The technical and/or operational nature of the deficiencies No. 1 and No. 2 provide objective evidence of serious failure of the implementation of the ISM code. An internal safety audit and corrective action is required within three months.
