The shutoff valve at the Fuel tank must be manually operable from outside the space. The STBD Fuel tank valve has a stripped handle nut and is questionable if completely closed with the others.

Must be provided with bilge pump. Unable to test bilge pump. Clean out pump and lines and prove proper operation to attending USCG Marine Inspector.

Repairs must be inspected by a marine inspector. The following were noted as work list items during the vessel's scheduled dry dock and must be verified complete to get DD/ISE credit. Work list items: Legs/Towers: -Port Leg tower box bottom is wasted. Provided satisfactory repair plan proposal prior to conducting repair. -Forward leg upper tower stiffeners wasted. Crop/renew. Skegs: -Remove doublers/temp repairs from port and starboard skegs – crop/ renew to good metal. -Weld on the upper inboard side of the starboard skeg has weep hole. Repair. -Crop/renew the wear plate for starboard skeg/strut. -Reinstall dry dock plugs near bottom of skegs. -Propose NDT (pneumatic air test, etc.) skegs to ensure watertight integrity upon final repairs. Gauging for the areas discussed. Provide report: -Forward and Port leg tower box bottoms. -Grid on aft bottom plating from port to starboard and stern side shell. -Grid on bottom plating IVO of forward hole -Grid on starboard side shell (several small holes) Bottom Plating/Hull: -Port aft insert that is already been fit up and welded in - gouge/re-weld locations shown with porosity and radius corner that was marked. NDT. -Aft internal longitudinals. Crop/renew wasted sections of longitudinals after gauging. -Area identified for bottom insert with epoxy temp repair aft/IVO of starboard skeg. Crop/renew to good metal. NDT. -Bottom shell hole near bow. Crop/ renew to good metal. Deck: -Hole in overhead starboard and forward with temp epoxy repair. Crop/Renew to good metal. NDT. -Clad weld temp. repair in overhead – starboard and forward. Crop/Renew to good metal. NDT. -Wastage under starboard bit (aft of house) to include area of deck plat with old through fitting for hose. Crop/renew to good metal. NDT. Starboard side shell: -Multiple small holes in upper side shell near deck weld FWD of the MDE. Crop renew to good metal after gauging area. NDT. Port Side shell: -Hole and temp repair (doubler) aft of leg/ tower. Crop/renew to good metal. NDT. -Doubler/wasted side shell IVO of galley sink drain. Crop/ renew to good metal. NDT. House: -Forward shell IVO of hydraulic supply/return line. Epoxy temp repair and hole. Crop/renew to good metal. Other: -Provide repair report for forward directional control valve. - Reattach hydraulic pipe hanger to overhead -Remove junction box with cable severed at both ends. 46 CFR 176-20-5 (Old T)

Repairs must be inspected by a marine inspector. At 02DEC24 DD/ISE the following work list item was issued: "Port Leg tower box bottom is wasted. Provide satisfactory repair plan proposal prior to conducting repair." After receipt of initial repair proposal and 05DEC24 attendance for visual inspection inside the port leg tower box, the work list item has been amended as follows for DD/ISE credit: -Visual inspection into the port leg tower box showed several wasted structural members/stiffeners within box. Crop all metal wasted below allowable limitations within box. Provide gauging report from certified NDT tech on metal within box not cropped to prove good metal. Provide new repair plan/ proposal from engineers for permanent repair based on findings 46 CFR 176-20-5 (Old T)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection. Valve fitted at tank connection was not able to be operated because valve handle was broken. Fix handle and ensure fuel shutoff works.

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order. crane used to launch workboat/rescue boat has hydraulic line that is not serviceable.

Sewage device must: be designed to prevent overboard discharge of treated or untreated sewage. Vessel has overboard discharge that was not locked closed.

Cranes: Operators: (a) the master or person in charge shall designate, in writing each crane operator

Every vessel shall be fitted with a suitable compass. Vessel had no date on compass deviation table. Must update compass deviation table.

Propulsion machinery shall be suitable for design. Port and starboard side engine engine had loose mechanical mechanisms for the clutch and throttle cables that could get snagged on machinary. Remove old clutch and throttle cables.

All vessels must have navigation lights in comliance with navigation rules. None of the navigation lights were operational. Must repair all navigation lights.

A vessel must be provided with a main steering gear that is Designed to operate at maximum astern speed without being damaged or jammed. Vessel steering tie rod is bent and is rubbing on vessel framing, causing misalignment and sticking of the rudders

Fuel lines must be annealed tubing of copper, nickel-copper, or copper-nickel having a minimum wall thickness of 0.9 millimeters (0.035 inch) except that Diesel fuel piping of other materials, such as seamless steel pipe or tubing, which provide equivalent safety may be used. Metal piped fuel line was found to have a flex hose with hose clamp in permanent use.

Fuel lines must be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration by the use of soft nonferrous metal straps which have no sharp edges and are insulated to protect against corrosion. Port MDE fuel filter is loosely attached to the engine and causing the attached hose to chafe.

A marine engineering system identified by the cognizant OCMI as being crucial to the survival of the vessel or to the protection of the personnel on board. Leak on lift boat hydraulic system found leaking at the PTO seal.

Exhaust gas must not leak from the piping or any connections. STBD engine exhaust leaking at flex and engine manifold.

The use of twist-on type wire nuts is permitted under the following conditions the connections must be made within an enclosure. Enclosure was improperly covered allowing access to the wiring.

Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. Bilge pump hose installed with one hose clamp in multiple locations.

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. Hand rail near STBD leg broke free from deck.

Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Anniversary date June, 21. Inspection completed October 1.

A marine engineering system identified by the cognizant OCMI as being crucial to the survival of the vessel or to the protection of the personnel on board. Hydraulic low level alarm required for liftboats by CG policy letter 07-02 failed to operate.

Replace six (06) missing pipe hangers/ supports for the hydraulic piping system.

Properly install main electrical cable where it enters the junction box, and at first bend where braided cable armor is chaffing against the frame.

Top course of STBD 01 deck rail was removed to accommodate crane when not in use or secured for transit. Install chain to serve as top course of railing and close gap when crane is in use (46 CFR 177.31).

Steering gear cable in way of port-side quadrant in engine room worn approximately 90%. Replace cable and prove proper operation of steering gear.

Install threaded plugs or caps in ends of three forward fuel tank drain lines (46 CFR 182.20).

Repair hole in port side engine room coaming (46 CFR 176.25).

Provide circuit breaker directory for distribution panel located in galley per 46 CFR 183.05.

Properly secure engine room egress ladders (46 CFR 177.15).

Install lagging on starboard diesel propulsion engine exhaust.

EPIRB hydrostatic release unit (HRU) is expired. Install approved HRU in accordance with 46 CFR 185.740.

Secure clutch oil piping with proper brackets.

Demonstrate proper operation of fuel shut-offs.

Repair/replace self-igniting floating electric waterlight for lifefloat.

Renew handle on shutoff valve for center fuel tank.

Reposition red & green nav lights to provide the proper field of view.

Re-route hose on vsl's bilge pump (stbd) to remove kink.

Repair or renew hanging emergency lights on fixture in engine room.

Replace exhaust lagging on both MDEs.

Install chaffing gear on fuel lines as indictaed.

Install proper pipe clamp on port water filter pipe.

Install metal clamps on electrical lines in e/r space.

Properly label all emergency escape exits in e/r.

Install safety light cover on range exhaust fixture over stove.

Repair hole on port side of galley bulkhead.

Replace broken safety light cover in main galley space.

Prior to sailing replace EPIRB battery or provide replacement EPIRB that has been registered.
