Repairs must be inspected by a marine inspector. The following were noted as work list items during the vessel's scheduled dry dock and must be verified complete to get DD/ISE credit. Work list items: Legs/Towers: -Port Leg tower box bottom is wasted. Provided satisfactory repair plan proposal prior to conducting repair. -Forward leg upper tower stiffeners wasted. Crop/renew. Skegs: -Remove doublers/temp repairs from port and starboard skegs – crop/ renew to good metal. -Weld on the upper inboard side of the starboard skeg has weep hole. Repair. -Crop/renew the wear plate for starboard skeg/strut. -Reinstall dry dock plugs near bottom of skegs. -Propose NDT (pneumatic air test, etc.) skegs to ensure watertight integrity upon final repairs. Gauging for the areas discussed. Provide report: -Forward and Port leg tower box bottoms. -Grid on aft bottom plating from port to starboard and stern side shell. -Grid on bottom plating IVO of forward hole -Grid on starboard side shell (several small holes) Bottom Plating/Hull: -Port aft insert that is already been fit up and welded in - gouge/re-weld locations shown with porosity and radius corner that was marked. NDT. -Aft internal longitudinals. Crop/renew wasted sections of longitudinals after gauging. -Area identified for bottom insert with epoxy temp repair aft/IVO of starboard skeg. Crop/renew to good metal. NDT. -Bottom shell hole near bow. Crop/ renew to good metal. Deck: -Hole in overhead starboard and forward with temp epoxy repair. Crop/Renew to good metal. NDT. -Clad weld temp. repair in overhead – starboard and forward. Crop/Renew to good metal. NDT. -Wastage under starboard bit (aft of house) to include area of deck plat with old through fitting for hose. Crop/renew to good metal. NDT. Starboard side shell: -Multiple small holes in upper side shell near deck weld FWD of the MDE. Crop renew to good metal after gauging area. NDT. Port Side shell: -Hole and temp repair (doubler) aft of leg/ tower. Crop/renew to good metal. NDT. -Doubler/wasted side shell IVO of galley sink drain. Crop/ renew to good metal. NDT. House: -Forward shell IVO of hydraulic supply/return line. Epoxy temp repair and hole. Crop/renew to good metal. Other: -Provide repair report for forward directional control valve. - Reattach hydraulic pipe hanger to overhead -Remove junction box with cable severed at both ends. 46 CFR 176-20-5 (Old T)

Repairs must be inspected by a marine inspector. At 02DEC24 DD/ISE the following work list item was issued: "Port Leg tower box bottom is wasted. Provide satisfactory repair plan proposal prior to conducting repair." After receipt of initial repair proposal and 05DEC24 attendance for visual inspection inside the port leg tower box, the work list item has been amended as follows for DD/ISE credit: -Visual inspection into the port leg tower box showed several wasted structural members/stiffeners within box. Crop all metal wasted below allowable limitations within box. Provide gauging report from certified NDT tech on metal within box not cropped to prove good metal. Provide new repair plan/ proposal from engineers for permanent repair based on findings 46 CFR 176-20-5 (Old T)
